摘  要:目的分析医学应用放射工作人员淋巴细胞微核,为放射工作人员健康监护提供科学依据。方法选择2019年1月1日-2020年12月31日参加职业健康检查的3884名在岗医学应用放射工作人员作为研究对象,对淋巴细胞微核检测结果进行分析与评价。结果放射工作人员微核率为(3.21±2.42)‰,微核阳性率为6.57%;微核率男性(3.12±2.40)‰低于女性(3.36±2.46)‰,差异有统计学意义(t=-2.96,P<0.05),微核阳性率男性低于女性,差异无统计学意义(χ^(2)=1.45,P>0.05);不同级别医疗机构组间比较,微核率、微核细胞率和微核阳性率由高到低依次为二级、一级及以下、三级,差异无统计学意义(F_(微核率)=0.739,F_(微核细胞率)=0.818,χ^(2)=1.505,P>0.05);不同工种组间比较,微核率和微核细胞率由高至低依次为核医学、介入放射学、牙科放射学、诊断放射学、放射治疗,差异有统计学意义(F_(微核率)=2.95,F_(微核细胞率)=3.195,P<0.05),微核阳性率由高到低依次为牙科放射学、核医学、介入放射学、诊断放射学、放射治疗,差异无统计学意义(χ^(2)=9.514,P>0.05);各年龄组间比较,≥50岁组微核率、微核细胞率和阳性率均高于其它年龄组,差异有统计学意义(F_(微核率)=94.74,F_(微核细胞率)=92.59,P<0.05),且随年龄增长呈增加趋势,有统计学意义(χ^(2)=142.97,P<0.05);各工龄组间比较,≤10年组的微核率、微核细胞率和微核阳性率均低于其它工龄组,差异有统计学意义(F_(微核率)=47.40,F_(微核细胞率)=47.65,χ^(2)=76.18,P<0.05),且随工龄增加均呈增长趋势,有统计学意义(χ^(2)=71.797,P<0.05)。结论甘肃省医学应用放射工作员淋巴细胞微核率和微核阳性率较高,在放射工作人员健康监护过程中应提示核医学、介入放射学和牙科放射学工作人员加强防护。Objective To analyze the lymphocyte micronucleus of medical radiation workers,and to provide scientific basis for the health monitoring of radiation workers.Methods A total of 3884 on-the-job medical radiation workers who participated in occupational health examinations from January 1,2019 to December 31,2020 were selected as the research subjects,and the results of lymphocyte micronucleus rate were analyzed and evaluated.Results The micronucleus rate of radiation workers was(3.21±2.42)‰,and the positive rate of micronucleus was 6.57%;the micronucleus rate of males(3.12±2.40)‰was lower than that of females(3.36±2.46)‰,and the difference was statistically significant(t=-2.96,P<0.05),the positive rate of micronucleus in males was lower than that in females,and the difference was not statistically significant(χ^(2)=1.45,P>0.05).There was no significant difference in the rate of nuclear abnormalities(Fmicronucleus rate=0.739,Fmicronucleate cell rate=0.818,χ^(2)=1.505,P>0.05);The micronucleus rate and micronucleus cell rate from high to low were nuclear medicine,interventional radiology,dental radiology,diagnostic radiology and radiotherapy,and the differences were statistically significant(t_(Fmicronucleus rate)=2.95,F_(micronucleus cell rate)=3.195,P<0.05).The positive rate of micronucleus from high to low was dental radiology,nuclear medicine,interventional radiology,diagnostic radiology,radiotherapy,and there was no significant difference(χ^(2)=9.514,P>0.05).The micronucleus rate,micronucleus cell rate and positive rate of micronucleus in≥50 years old group were higher than those in other age groups,and the differences were statistically significant(F_(micronucleus rate)=94.74,F_(micronucleus cell rate)=92.59,P<0.05),and it increased with age,with statistical significance(χ^(2)=142.97,P<0.05).The micronucleus rate,micronucleus cell rate and micronucleus positive rate of≤10 a group were lower than those of other working age groups,and the differences were statistically significant(Fmicronucleus rat
