天麻钩藤饮加减治疗肝阳上亢型后半规管耳石症复位后残余眩晕的疗效评价

摘　　要：目的 观察天麻钩藤饮加减治疗肝阳上亢型后半规管耳石症复位后残余眩晕的临床疗效。方法 将2021年1月—2022年2月辽宁中医药大学附属医院收治的60例肝阳上亢型后半规管耳石症复位后伴有残余眩晕的患者,随机分为对照组和治疗组各30例。对照组给予银杏叶提取物片治疗,治疗组给予天麻钩藤饮加减治疗,均治疗2周。比较两组治疗前和治疗后1、2周,1个月的中医证候积分、眩晕障碍量表(DHI)评分、视觉模拟(VAS)评分,焦虑抑郁量表(HADS)的评分及治疗2周后的疗效。结果 两组治疗后中医证候积分、DHI评分(DHI-P、DHI-E、DHI-F评分、DHI总分)、VAS评分、HADS评分均有明显改善。治疗后2周、1个月时,治疗组DHI评分(DHI-P、DHI-E、DHI-F评分、DHI总分)、VAS评分、HADS评分均明显低于对照组(P<0.05)。治疗后2周,治疗组的疗效优于对照组(P<0.05)。结论 天麻钩藤饮加减治疗肝阳上亢型后半规管耳石症复位后残余眩晕疗效较好,值得临床推广。Objective To observe the clinical effect of Tianma Gouteng Yin(天麻钧藤饮)on residual dizziness after reduction of posterior semicircular otolith of hyperactivity of liver yang.Methods From January 2021 to February 2022,60 patients with residual dizziness after successful reduction of post semicircular otolith of hyperactivity of liver Yang of the Affiliated Hospital of Liaoning University of traditional Chinese medicine were randomLy divided into control group(n=30)and treatment group(n=30).The control group was treated with Ginkgo Biloba Extract Tablets;The treatment group was treated with Tianma Gouteng Yin.The course of treatment of both groups was 2 weeks.The scores of traditional Chinese medicine syndrome,dizziness Disorder Inventory(DHI),visual simulation score(VAS),hospital anxiety and Depression Scale(HADS)and the curative effect after 2 weeks of treatment were compared between the two groups before treatment and 1 week,2 weeks and 1 month after treatment.Results After treatment,the traditional Chinese medicine syndrome score,DHI score(DHI-P,DHI-E,DHI-F score and total DHI score),VAS score and hads score of the two groups were significantly improved.At 2 weeks and 1 month after treatment,the DHI scores(DHI-P,DHI-E,DHI-F scores and total DHI scores),VAS scores and hads scores of the treatment group were significantly lower than those of the control group(P<0.05).2 weeks after treatment,the curative effect of the treatment group was better than that of the control group(P<0.05).Conclusion Tianma Gouteng Yin has a good effect in the treatment of residual vertigo after reduction of posterior semicircular otolith of hyperactivity of liver Yang,which is worthy of clinical promotion.
