活瘀祛浊益气方治疗特发性膜性肾病(脾肾气虚、浊瘀内阻)临床观察

摘　　要：目的 探讨活瘀祛浊益气方治疗特发性膜性肾病(脾肾气虚、浊瘀内阻)患者的临床疗效及安全性。方法 选取2019年1月—2021年1月驻马店市中医院肾病科收治的特发性膜性肾病患者(脾肾气虚、浊瘀内阻)128例,根据随机数字表法分为两组,对照组64例采用常规西药联合他克莫司胶囊治疗,观察组64例基于对照组治疗方法加用活瘀祛浊益气中药汤剂治疗,比较两组患者的临床疗效及安全性。结果 治疗后,观察组患者治疗的总有效率为85.9%,复发率为29.1%,与对照组的总有效率67.2%、复发率48.8%比较,差异有统计学意义(P<0.05)。两组治疗前24 h尿蛋白、血浆白蛋白、血肌酐、尿素氮及甘油三脂和血清总胆固醇水平比较均差异无统计学意义(P>0.05),治疗后两组患者各指标均较治疗前明显改善(P<0.05),且观察组指标明显优于对照组,差异有统计学意义(P<0.05)。结论 中西医结合治疗特发性膜性肾病(脾肾气虚、浊瘀内阻证)临床效果较单用西药治疗效果满意,对改善患者症状及脂代谢等内环境均有明显效果,而且对蛋白尿的丢失具有一定的控制作用,且安全性好。Objective To investigate the clinical effect of Huoyu Quzhuo Yiqi Decoction(活瘀祛浊益气方)in the treatment of idiopathic membranous nephropathy(Qi deficiency of spleen and kidney,internal obstruction of phlegm and turbid).Methods A total of 128 patients with idiopathic membranous nephropathy(Qi deficiency of spleen and kidney,internal obstruction of phlegm and turbid)admitted to the Department of Nephrology,Zhumadian Traditional Chinese Medicine Hospital from January 2019 to January 2021 were selected and divided into two groups according to the random number table method.The control group(64 cases)was treated with conventional western medicine combined with Tacrolimus Capsules,and the observation group(64 cases)was treated with Huoyu Quzhuo Yiqi Decoction based on the treatment method of the control group.The clinical efficacy and safety of the two groups were compared.Results After treatment,the total effective rate of the control group was 67.2%,the recurrence rate was 48.8%.Compared with those(85.9%,29.1%)of the observation group,the difference was statistically significant(P<0.05).Before treatment,24 h urinary protein,serum albumin,serum creatinine,urea nitrogen and three triglyceride and serum total cholesterol level of the two groups showed no significant difference(P>0.05).After treatment,each index of two groups was significantly improved(P<0.05),and those of the observation group were improved significantly better than thos eof the control group(P<0.05).Conclusion Huoyu Quzhuo Yiqi Decoction in the treatment of idiopathic membranous nephropathy has a good clinical effect.It has obvious effects on improving the symptoms and lipid metabolism of the patients,and has a certain control effect on the loss of protein urine.
