冰冻切片对复杂性肾囊性病变的诊断价值分析

摘　　要：目的探究冰冻切片对复杂性肾囊性病变的诊断价值。方法选取2017年1月至2018年1月于本院行冰冻切片检查的77例复杂性肾囊性病变患者作为研究对象。患者均行冰冻切片及石蜡切片检查,以术后石蜡切片病理诊断为金标准,统计石蜡切片统计结果,分析冰冻切片对复杂性肾囊性病变的诊断效能。结果77例患者中,石蜡切片诊断为恶性肿瘤病变45例(58.44%),其中肾乳头状腺癌8例(10.39%),肾透明细胞癌33例(42.86%),肾多房囊性肾细胞癌4例(5.19%);良性病变32例(41.56%),其中肾囊肿18例(23.38%),肾肿瘤为13例(16.88%),肾结核为1例(1.30%)。77例患者经冰冻切片检查,灵敏度为93.33%,特异度为96.88%,准确度为94.81%;经一致性检验,冰冻切片与金标准一致性较好(Kappa=0.894)。结论冰冻切片对复杂性肾囊性病变的诊断效能较高,能为临床诊断提供更客观的依据,值得临床应用推广。Objective To explore the diagnostic value of frozen section in complicated renal cystic lesions.Methods 77 patients with complex renal cystic lesions who underwent frozen section examination in our hospital from January 2017 to January 2018 were selected as the study subjects.All patients underwent frozen section and paraffin section examination.The pathological diagnosis of postoperative paraffin section was regarded as the gold standard,and the statistical results of paraffin section were counted,the efficacy of frozen section in diagnosis of complex renal cystic diseases.Results Of the 77 patients,45(58.44%)were diagnosed as malignant tumors by paraffin section,including 8(10.39%)papillary adenocarcinoma of kidney,33(42.86%)clear cell carcinoma of kidney,and 4(5.19%)multilocular cystic renal cell carcinoma of kidney.There were 32 benign lesions(41.56%),of which 18 were renal cysts(23.38%),13 were renal tumors(16.88%),and 1 was renal tuberculosis(1.30%).The sensitivity,specificity and accuracy of 77 patients were 93.33%,96.88%and 94.81%respectively.The consistency test showed that the frozen section was in good agreement with the gold standard(Kappa=0.894).Conclusion Frozen section has high diagnostic efficiency for complex renal cystic diseases,and can provide more objective basis for clinical diagnosis,which is worthy of clinical promotion and application.
