血清糖类抗原125、B型脑钠肽联合和肽素检测在心力衰竭伴心房纤颤患者中的临床意义

摘　　要：目的:分析血清糖类抗原125(CA125)、B型脑钠肽(BNP)及和肽素联合检测,在心力衰竭伴心房纤颤(AF)中的临床意义.方法:选取2020年1月至2022年1月本院收治的心力衰竭合并AF患者71例为研究对象,纳入观察组.根据随访期间观察组患者病情发生情况,又将其分为预后良好亚组(n=38)与预后不良亚组(n=33).另选取同期本院收治的单纯心力衰竭患者88例纳入对照组.比较观察组与对照组之间及观察组中预后良好亚组与预后不良亚组之间CA125、BNP及和肽素表达水平,进一步分析CA125、BNP及和肽素对心力衰竭合并AF的诊断价值.结果:观察组CA125、BNP及和肽素水平高于对照组,差异有统计学意义(P<0.05);观察组中预后不良亚组CA125、BNP及和肽素水平明显高于预后良好亚组,差异亦有统计学意义(P<0.05).CA125、BNP及和肽素联合检测心力衰竭合并AF诊断特异度明显高于3者单独诊断,差异有统计学意义(P<0.05).结论:CA125、BNP及和肽素在心力衰竭伴AF患者中呈现高表达,且与患者预后密切相关,在患者早期诊断中具有较高的应用价值.Objective:To analyze the clinical significance of combined detection of serum carbohydrate antigen 125(CA125),B-type brain natriuretic peptide(BNP),and copeptin in heart failure with atrial fibrllation(AF).Methods:Seventy-one patients with heart failure combined with AF were treated in our hospital from January 2020 to January 2022 were selected as research objects and included in observation group.According to the patient's condition during the follow-up period,among research objects in observation group were further divided into a good prognosis sub-group(n=38)and a poor prognosis sub-group(n=33).And another 88 patients with heart failure without AF treated in the same period and the same hospital were selected as the control group.The expression levels of CA125,BNP,and copeptin in patients were compared between observation sub-group and control group and between poor prognosis subgroup and good prognosis sub-group.And the diagnostic value of CA125,BNP and copeptin in heart failure complicated with AF were further analyzed.Results:The levels of CA125,BNP and copeptin in the observation group were higher than those in the control group(P<0.05);the levels of CA125,BNP,and copeptin in the observation group of the poor prognosis sub-group were significantly higher than those of the good prognosis sub-group(P<0.05).The diagnostic specificity of combined detection of CA125,BNP and copeptin for heart failure combined with AF was significantly higher than that of one of the three paramters(P<0.05).Conclusion:CA125,BNP and copeptin are highly expressed in patients with heart failure complicated with AF,and are closely related to the prognosis of patients,which have high application value in early diagnosis of patients.
