“从肝论治”儿童咳嗽变异性哮喘的辨治思路

摘　　要：儿童咳嗽变异性哮喘(cough variant asthma,CVA)为儿科临床常见疾病,在我国儿童慢性咳嗽所占比例日渐升高。基于中医理论中肝肺两脏腑之间调畅气机的生理功能,以及两脏腑通过中医经络学说中经络连接的密切性,结合儿童咳嗽变异性哮喘反复发作,阵发性咳嗽的疾病特点以及临床经验。总结出CVA风邪犯肺,肝郁气逆的病因病机。根据病因提出疏风宣肺,调肝理气的治疗总则。在此基础上,依照临床中患儿的病症有所偏盛进行疏风宣肺及调肝理肺进行不同的侧重选择。对“从肝论治”咳嗽变异性哮喘的诊疗思路进行了病因病机,治法方药以及预防发作方面的总结。Cough variant asthma in children is a common disease in pediatric clinics.And the proportion of chronic cough in children is increasing in China.In this paper,based on the physiological function of the liver and lung organs in regulating qi flow and the close connection between the two organs through the meridians in traditional Chinese medicine,we combined the disease characteristics of recurrent attacks of cough variant asthma and paroxysmal cough in children with cough and clinical experience.He summarized the etiology and pathogenesis of CVA wind evil offending the lung and liver depression and Qi rebellion.According to the etiology,we proposed the general treatment principle of draining wind and promoting lung and regulating liver and Qi.On the basis of this,different emphasis was placed on draining the wind and promoting the lung and regulating the liver and lung according to the prevalence of the disease in clinical children.Finally,we summarized the diagnosis and treatment of cough variant asthma in terms of etiology,treatment,prescription and prevention of attacks.
