以络病理论为指导探讨围绝经期头痛的诊疗

摘　　要：络及络脉的概念首见于《黄帝内经》,经过历代医家的发展与补充,络病理论体系已逐步完善,至现代,更多的医家开始重视并研究络病理论,应用在诸多疾病治疗中。围绝经期头痛属于“脑络病”,文章从病因、病机、临床特点等方面对比围绝经期头痛与络病的相似之处,用络病理论来解释围绝经期头痛。围绝经期头痛为本虚标实之证,此时天癸已竭,肾阴不足,加之肝阳上扰,使络脉瘀阻。叶天士在《临证指南医案》中提出了:“络以通为用”的基本治则,根据围绝经期头痛的病因,病机等以“络以通为用”的基本治则为指导采用平肝活血通络、滋肾化瘀通络、补虚荣络等方法为围绝经期头痛的治疗提供新的方向。Collateral and collateral disease theory was first introduced in the book"Huangdi Neijing",and through the development and supplementation of various generations of medical doctors,the theory system of collateral disease theory has been gradually improved.By modern times,more and more physicians are paying attention to and refining collateral disease theory.Application of collateral disease theory in the treatment of many diseases.Perimenopausal headache belongs to the category of“brain colla-teral disease”.This paper discusses the similarities between perimenopausal headache and“collateral disease”from the aspects of cause,course of disease,scope of lesion,characteristics of pathogenesis and clinical manifestations.Explanation of perimenopausal headache by collateral disease theory.Perimenopausal headache is deficiency in root and excess in branches,in this period,Insufficient kidney yin,combined with liver yang disturbance,causing obstruction of collaterals.Ye Tianshi’s Linzheng Zhinan Yi’an puts forward the common treatment method of collateral disease:“unblocking the collaterals”.So in terms of treatment,according to the etiology,pathogenesis and disease characteristics of perimenopausal headache,we advocate the basic treatment principle of“unblocking the collaterals”,and the corresponding therapeutic methods of calming the liver and invigorating the blood,tonifying the kidney and removing blood stasis,and tonifying deficient collaterals in hope to provide new ideas and directions for the treatment of perimenopausal headache.
