基于“吴门医派络病理论”益肾活络方治疗慢性肾脏病3期的疗效观察

摘　　要：目的基于“吴门医派络病”理论,观察“益肾活络方”对CKD3期患者的临床疗效。方法选择2016年1月—2019年10月苏州市中医医院100例慢性肾脏病3期患者,随机纳入治疗组和对照组,治疗组给予具有“益肾活络方”+基础治疗,对照组给予基础治疗。观察治疗4、12、24周后疗效性指标。结果治疗组总有效率91.1%高于对照组的84.8%(P<0.05);中医证候总积分治疗组有明显改善(P<0.05),对照组无明显改善(P>0.05);UTP、Scr治疗组治疗4周,无统计学意义(P>0.05),治疗12、24周后,有统计学意义(P<0.05);对照组无统计学意义(P>0.05);eGFR治疗组治疗4、12周,无统计学意义(P>0.05),治疗24周,有统计学意义(P<0.05);对照组无统计学意义(P>0.05)。结论“益肾活络方”可以改善慢性肾脏病3期肾虚湿瘀证患者临床症状,降低CKD3期患者24 h尿蛋白定量、血肌酐水平、提高肾小球滤过率,在一定程度上延缓肾功能恶化。Objective To observe the clinical effect of Yishen-Huoluo prescription on patients with chronic kidney disease(CKD3)based on the theory of"Wumen medicine sending collateral disease".Methods 100 patients with stage 3 chronic kidney disease in Suzhou Hospital of Traditional Chinese Medicine from January 2016 to October 2019 were selected and randomly included into the treatment group and control group.The treatment group was given"Yishen-Huoluo formula"+basic treatment,and the control group was given basic treatment.The therapeutic effect indexes were observed after 4,12 and 24 weeks of treatment.Results The total effective rate of the treatment group was 91.1%higher than that of the control group(84.8%)(whether there was statistical significance).The total score of traditional Chinese medicine syndrome was significantly improved in the treatment group(P<0.01),but not in the control group(P>0.05).UTP and Scr:4 weeks after treatment,there was no statistical significance(P>0.05),12 weeks after treatment,there was statistical significance(P<0.05),24 weeks after treatment,there was statistical significance(P<0.01);there was no statistical significance in control group(P>0.05).eGFR:After 4 and 12 weeks of treatment,there was no statistical significance in the treatment group(P>0.05),and after 24 weeks,there was statistical significance(P<0.05),there was no statistical significance in control group(P>0.05).Conclusion"Yishen-Huoluofang"can improve the clinical symptoms of stage 3 chronic kidney disease patients with kidney deficiency and dampness-stasis syndrome,reduce the 24-hour urinary protein quantity,serum creatinine level and increase glomerular filtration rate in stage CKD3 patients,and to some extent delay the deterioration of renal function.
