保胎汤配合低剂量地屈孕酮治疗黄体功能不全型先兆流产的临床观察

摘　　要：目的探讨对黄体功能不全型先兆流产的患者配合使用低剂量地屈孕酮加保胎汤的效果和价值。方法选择2021年2月—2022年6月濮阳市妇幼保健院收治的黄体功能不全型先兆流产的68例患者作为本次研究对象,采用随机分组的方式,分为研究组和对照组两组各34例。对照组采用低剂量的屈孕酮治疗,研究组在低剂量的屈孕酮的基础上配合使用保胎汤进行治疗。从两组患者治疗后的临床症状缓解时间、血清激素的水平指标、出现血清抗体阳性和患者对药物的不良反应情况四个方面,比较两组患者在不同治疗后的治疗效果。结果患者在配合使用保胎汤加低剂量地屈孕酮治疗后的研究组在腹痛、腰酸、止血、住院时间和人绒毛膜促性腺激素(HCG)、孕酮(P)、雌二醇(E2)的激素水平指标均优于使用低剂量地屈孕酮治疗后的对照组的患者,即研究组的临床症状缓解时间、血清激素的水平指标均优于对照组,且研究组患者治疗后出现血清抗体阳性和患者对药物的不良反应情况也比对照组患者治疗后出现血清抗体阳性和患者对药物的不良反应情况少(P<0.05)。结论对黄体功能不全型先兆流产的患者使用保胎汤加低剂量地屈孕酮的药物进行治疗,能缩短患者的临床症状缓解时间,提高患者治疗后的血清激素的水平指标,减少患者在治疗过程中出现的不良反应情况。Objective To explore the clinical application effect and value of low-dose dextran plus Baotai Decoction(保胎汤)for patients with luteal insufficiency type of preeclampsia.Methods Eighty patients with luteal insufficiency with preeclampsia admitted to Puyang Maternal and Child Health Hospital from February 2021 to June 2022 were selected as the subjects of this study,and the subjects of this study were divided into two groups,the study group and the control group,with 34 cases in each group.The control group was treated with a single low-dose dydrogesterone,while the study group was treated with low-dose dydrogesterone together with luteal insufficiency preeclampsia soup.The therapeutic effects of the two groups after different treatments were compared in four aspects:the time of clinical symptom relief,the level index of serum hormones,the appearance of positive serum antibodies and the patients'adverse reactions to drugs.Results The abdominal pain,lumbago,hemostasis,length of hospital stay and the indicators of hormone levels of human chorionic gonadotropin(HCG),progesterone(P)and estradiol(E2)in the study group were better than those in the control group after treatment with low-dose drospirenone,i.e.,the time to relief of clinical symptoms and the indicators of serum hormone levels in the study group were better than those in the control group.In addition,the patients in the study group showed less serum antibody positivity and adverse reactions to drugs after treatment than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of patients with luteal insufficiency preeclampsia with the use of fetus preservation soup plus low-dose dydrogesterone can shorten the time of clinical symptom relief,improve the index of serum hormone level after treatment,and reduce the adverse reactions of patients in the process of treatment,which is worthy of application and promotion.
