桡动脉掌浅支皮瓣术、逆行岛状皮瓣术对指端皮肤缺损患者恢复情况、手指外观及手指功能评分的影响

摘　　要：目的:探讨对指端皮肤缺损患者采用桡动脉掌浅支皮瓣术、逆行岛状皮瓣术治疗的修复效果。方法:选取2021年4月—2022年12月毕节市第一人民医院收治的90例指端皮肤缺损患者作为研究对象,随机分为两组。对照组采用逆行岛状皮瓣术治疗,研究组采用桡动脉掌浅支皮瓣术,比较两组围手术期相关指标,术后恢复情况,手指外观和手指功能恢复情况。结果:研究组手术时间、住院时间均短于对照组,术后指间关节活动度均高于术前和对照组术后(P<0.05)。术后研究组外观不满意率、痛觉敏感率、感觉麻木率、两点辨别觉检查试验指标均优于对照组(P<0.05)。术后1个月和6个月研究组手指外观与功能评分均高于对照组(P<0.05)。术后6个月,研究组手指感觉、血液循环、恢复工作情况评分均高于对照组(P<0.05)。结论:对于指端皮肤缺损患者采用桡动脉掌浅支皮瓣术治疗,相较于逆行岛状皮瓣术,能更好地改善围手术期指标,恢复手指功能,加快康复进程。Objective:To explore the repair effect of radial artery superficial palm branch flap surgery and retrograde island flap surgery on patients with finger tip skin defect.Method:A total of 90 patients with finger tip skin defect admitted to the First People's Hospital of Bijie City from April 2021 to December 2022 were selected as the research objects and randomly divided into two groups.The control group received retrograde island flap surgery,while the study group received radial artery superficial palm branch flap surgery.The perioperative related indicators,postoperative recovery,the appearance and functional recovery of the fingers of the two groups of patients were compared.Result:The surgical time and hospitalization time of the study group were shorter than those of the control group,and the range of motion of the interphalangeal joint after surgery was higher than that before and that after surgery in the control group(P<0.05).After surgery,the appearance dissatisfaction rate,pain sensitivity rate,sensory numbness rate and two point discrimination test indicators of the study group were all better than those of the control group(P<0.05).The appearance and function scores of the fingers in the study group were higher than those in the control group at 1 and 6 months after surgery(P<0.05).6 months after surgery,the scores of finger sensation,blood circulation and recovery of work in the study group were higher than those in the control group(P<0.05).Conclusion:Compared with retrograde island flap surgery,the application of radial artery superficial palm branch flap surgery in patients with finger tip skin defect can improve perioperative indicators,restore finger function and accelerate the recovery process.
