参芪利水汤联合腕踝针治疗乳腺癌术后淋巴水肿的临床对照研究

摘　　要：目的:探讨参芪利水汤联合腕踝针在乳腺癌术后淋巴水肿患者中的应用价值。方法:选择2020年7月—2022年8月江西省肿瘤医院收治的乳腺癌术后淋巴水肿患者120例,按随机数字表法分为两组,各60例。对照组采用常规西医治疗,试验组在对照组基础上采用参芪利水汤联合腕踝针治疗。对比两组炎症反应、中医症候积分、患肢症状评分、肿胀程度、症状困扰指数、上肢功能、临床疗效、安全性。结果:治疗后,试验组C反应蛋白(CRP)、白细胞介素(IL)-6、基质金属蛋白酶2(MMP-2)水平和中医症候积分、患肢症状评分及肿胀程度、乳腺癌淋巴水肿症状体验指数(BCLE-SEI)、上肢功能评定表(DASH)评分均低于对照组,治疗总有效率高于对照组,差异均有统计学意义(P<0.05);两组均未出现不良反应。结论:参芪利水汤联合腕踝针能够有效减轻乳腺癌术后淋巴水肿患者患肢肿胀,下调CRP、IL-6、MMP-2水平,减轻患者症状及困扰,促进上肢功能恢复,且安全性较高。Objective:To investigate the application value of Shenqi Lishui Decoction combined with wrist-ankle acupuncture in patients with lymphedema after breast cancer surgery.Method:A total of 120 patients with lymphedema after breast cancer surgery treated in Jiangxi Cancer Hospital from July 2020 to August 2022 were selected and divided into two groups according to random number table method,with 60 patients in each group.The control group was treated with conventional western medicine,and the experimental group was treated with Shenqi Lishui Decoction combined with wrist-ankle acupuncture on the basis of the control group.The inflammatory response,TCM symptom score,affected limb symptom score,swelling degree,symptom distress index,upper limb function clinical efficacy and safety were compared between the two groups.Result:After treatment,the levels of C reactive protein(CRP),interleukin(IL)-6,matrix metalloproteinase 2(MMP-2),TCM symptom score,affected limb symptom score,swelling degree,breast cancer and lymphedema symptom experience index(BCLE-SEI)and disabilities of the arm,shoulder and hand(DASH)score in experimental group were lower than those in control group,and the total effective rate of treatment in experimental group was higher than that in control group,the differences were statistically significant(P<0.05).There were no adverse reactions in both groups.Conclusion:Shenqi Lishui Decoction combined with wrist-ankle acupuncture can effectively reduce the swelling of affected limbs in patients with lymphedema after breast cancer surgery,reduce the levels of CRP,IL-6 and MMP-2,reduce the symptoms and distress of patients,and promote the recovery of upper limb function,with high safety.
