摘　　要：疱疹样皮炎(dermatitis herpetiformis,DH)是一种罕见的慢性易复发性自身免疫性大疱性皮肤病,是肠道疾病乳糜泻的特有的肠外皮肤表现,属于谷胶敏感性疾病。亚洲人群比较少见,多发于高加索人群,国内也可见少数病例报道。DH发病与人类白细胞抗原(human leukocyte antigen,HLA)相关基因密切相关,但具体的发病机制尚未完全阐明。目前国内外有关DH尚无统一的循证指南和诊断标准,给临床医师的诊疗活动带来一定难度。无谷胶饮食是治疗DH的关键,氨苯砜被认定为治疗DH的首选药物,替代治疗和免疫治疗也逐渐进入人们的视野。本文就DH的研究进展进行综述,以期加深人们对DH的认识。Dermatitis herpetiformis(DH)is a rare,chronic and recurrent autoimmune bullous dermatosis,which is a unique extraintestinal skin manifestation of intestinal disease celiac disease and belongs to glutensensitive disease.The Asian population is relatively rare,mostly in the Caucasian population,and a few caseshave been reported in China.The pathogenesis of DH is closely related to human leukocyte antigen(HLA)related genes,but the specific pathogenesis has not been fully elucidated.At present,there are no unified evidence-based guidelines and diagnostic criteria for DH at home and abroad,which brings some difficulties to the clinical diagnosis and treatment.Gluten-free diet is the key to the treatment of DH,Dapsone has been identified as the first choice for the treatment of DH,and replacement therapy and immunotherapy have gradually entered people's field of vision.This paper reviews the research progress of DH in order to deepen people's understanding of DH.
