中药熏蒸治疗婴儿肛周脓肿临床观察

摘　　要：目的研究中药熏蒸在婴儿肛周脓肿的中西医疗效和预后。方法回顾性分析赣州市妇幼保健院2018年1月—2021年12月收治的83例肛周脓肿患儿的临床资料,其中观察组(中药熏蒸)54例、对照组(高锰酸钾)29例。比较中药熏蒸和高锰酸钾坐浴对患儿中医症状评分、术口愈合时间等指标的影响,分析术口愈合时间和脓肿复发结局的影响因素。结果观察组患儿治疗后的中医症状评分下降程度较对照组大(P<0.05)。观察组术口愈合时间较对照组更短(P=0.001)。多因素回归分析发现,中药熏蒸可缩短术口愈合时间(P=0.004),降低肛周脓肿复发风险(HR=0.12,95%CI 0.03~0.39,P=0.001),而脓肿直径是肛周脓肿复发的风险因素(HR=1.65,95%CI 1.39~1.96,P<0.001)。结论中药熏蒸可以提高婴儿肛周脓肿的治疗效果,缩短术口愈合时间,降低肛周脓肿的复发风险。Objective To study the therapeutic effect and prognosis of Chinese medicine fumigation in perianal abscess.Methods Retrospective The clinical data of 84 infants with perianal abscess treated in Ganzhou Maternal and Child Health Care Hospital from January 2018 to December 2021 were retrospectively analyzed,including 54 cases in the observation group(traditional Chinese medicine fumigation)and 29 cases in the control group(potassium permanganate).The effects of Chinese medicine fumigation and potassium permanganate hip bath on TCM symptom score,operative healing time and other indexes was compared,and the influencing factors of operative healing time and the outcome of abscess recurrence was analyzed.Results After treatment,the Chinese medicine symptom score of the observation group decreased more than that of the control group(P<0.05).The healing time of observation group was shorter than that of control group(P=0.001).Multivariate regression analysis showed that traditional Chinese medicine fumigation could shorten the healing time of the operative mouth(P=0.004)and reduce the risk of recurrence of perianal abscess(HR=0.12,95%CI 0.03~0.39,P=0.001),and the diameter of abscess was a risk factor for recurrence of perianal abscess(HR=1.65,95%CI 1.39~1.96,P<0.001).Conclusion Chinese medicine fumigation can improve the therapeutic effect of infantile perianal abscess,shorten the healing time of operation mouth,and reduce the recurrence risk of perianal abscess.
