空心钉结合带缝匠肌骨瓣移植治疗青壮年移位股骨颈骨折的效果

摘　　要：目的探究空心钉结合带缝匠肌骨瓣移植治疗青壮年移位股骨颈骨折(FNF)的临床效果。方法将80例青壮年移位FNF患者按随机数字表法分为对照组与观察组,每组40例。对照组采用单纯空心钉内固定术治疗,观察组采用空心钉结合带缝匠肌骨瓣移植治疗。比较2组手术与恢复相关指标、骨折愈合率、股骨头缺血性坏死(AVN)率及术前术后6个月髋关节功能[采用髋关节评分量表(Harris)评价]。结果观察组手术时间、骨折愈合率、术后6个月Harris评分均较对照组显著增加,骨折愈合时间、住院时间均较对照组显著减少,差异均有统计学意义(P<0.05)。2组术中出血量、AVN率比较差异均无统计学意义(P>0.05)。结论空心钉结合带缝匠肌骨瓣移植治疗青壮年移位FNF,虽然手术时间延长,但可促进骨折愈合,提高患者髋关节功能。Objective To investigate the clinical efficacy of cannulated screwfixation combined with sartorius bone flap transplantation in the treatment of displaced femoral neck fracture(FNF)in young adults.Methods Eighty young adults with displaced FNF were randomlytreated with cannulated screw fixation alone(control group,40 cases)or in combination with sartorius bone flap transplantation(observation group,40 cases).Operation and recovery indexes,fracture healing rate,and avascular necrosis(AVN)rate of femoral head were compared between the two groups.In addition,hip function was evaluated using the Harris scale before and 6 months after operation.Results Compared with the control group,operation time was prolonged,fracture healing rate and Harris score at 6 months after operation were increased,and fracture healing time and hospital stay were shortened in the observation group(P<0.05).There were no significant differences in intraoperative blood loss and AVN rate between the two groups(P>0.05).Conclusion Cannulated screw fixation combined with sartorius bone flap transplantation for displaced FNF in young adults can promote fracture healing and improve hip function,although the operation time is prolonged.
