摘　　要：目的分析宫腔镜冷刀术后子宫内膜息肉复发的影响因素,为子宫内膜息肉患者治疗提供参考依据。方法选取2018年5月至2020年1月在南昌市第一医院诊治的197例子宫内膜息肉并行宫腔镜冷刀治疗的患者为研究对象,收集临床资料[包括患者年龄、BMI、孕次、产次、流产史、痛经情况、绝经状态、息肉直径、息肉数量、合并妇科疾病情况(盆腔炎、子宫肌瘤、多囊卵巢综合征等)、术后孕激素治疗和合并高血压、糖尿病情况],并对患者宫腔镜术后1年子宫内膜息肉的复发情况进行随访,采用单因素和多因素分析探讨子宫内膜息肉患者宫腔镜冷刀术后复发的影响因素。结果宫腔镜冷刀治疗后子宫内膜息肉复发率为17.25%(34/197)。单因素分析提示年龄、体重指数(BMI)、息肉数量、术后孕激素治疗与宫腔镜冷刀术后子宫内膜息肉复发存在相关性(P<0.05);多因素分析提示年龄、BMI、息肉数量、术后孕激素治疗是影响子宫内膜息肉患者宫腔镜术后复发的独立因素(P<0.05)。结论年龄、BMI、息肉数量、术后孕激素药物治疗是子宫内膜息肉复发的相关影响因素。Objective To analyze the factors affecting the recurrence of endometrial polyps after hysteroscopic cold knife surgery,and to provide guidance for the treatment of endometrial polyps.Methods A total of 197 patients with endometrial polyps who were diagnosed and treated with hysteroscopic cold knife in the First Hospital of Nanchang from May 2018 to January 2020 were selected as the subjects.The clinical data were collected,including age,body mass index(BMI),number of pregnancies,number of deliveries,abortion history,dysmenorrhea,menopausal status,polyp diameter,polyp number,gynecological diseases(pelvic inflammatory disease,uterine fibroids,polycystic ovary syndrome,etc.),postoperative progesterone treatment,hypertension,and diabetes mellitus.These patients were followed up for 1 year,and the factors influencing endometrial polyp recurrence were investigated using univariate and multivariate analysis.Results The recurrence rate of endometrial polyps was 17.25%(34/197)after hysteroscopic cold knife treatment.Univariate analysis showed that age,BMI,polyp number and postoperative progesterone therapy were correlated with endometrial polyp recurrence after hysteroscopic cold knife surgery(P<0.05).Multivariate analysis showed that age,BMI,polyp number and postoperative progesterone therapy were independent factors for the recurrence of endometrial polyps(P<0.05).Conclusion Age,BMI,polyp number and postoperative progesterone therapy are the related factors influencing the recurrence of endometrial polyps.
