中西医结合治疗头面部带状疱疹的临床研究

摘　　要：目的头面部带状疱疹患者50%以三叉神经痛为主,多发生于>60岁患者,治疗不及时或治疗方法不当常导致带状疱疹后遗神经痛。本研究分析中西医结合治疗头面部带状疱疹的临床效果,为降低带状疱疹后遗神经痛的发生提供参考方案。方法选取2020年1月—2022年12月天津市东丽医院收治的20例头面部带状疱疹患者,分为轻、中、重型,给予阿昔洛韦、泛昔洛韦、伐昔洛韦、更昔洛韦抗病毒治疗;维生素B1、甲钴胺、神经妥乐平营养神经;尼美舒利、普瑞巴林、帕瑞昔布止痛。通过运用中西医结合的方法,进行辨证分型施治。再辅以拔火罐、中药外敷、红光、氦氖激光治疗。结果多数患者通过中西医结合治疗7 d后,患处水疱干涸结痂,20例患者疼痛明显减轻,16例患者10 d后疼痛消失,2例糖尿病患者疼痛时间持续1个月后逐渐康复。结论中西医结合治疗头面部带状疱疹有效率高,能明显降低后遗神经痛发生率,值得临床推广应用。Objective Trigeminal neuralgia was the main pain of 50%of patients with shingles in the head and face,which mostly occurs in patients over 60 years old.Delayed treatment or improper treatment method often led to postherpetic neuralgia.This study analyzed the clinical effect of integrated traditional Chinese and Western medicine on shingles in the head and face,and provided a reference plan to reduce the incidence of postherpetic neuralgia.Methods Twenty patients with head and face shingles admitted to Tianjin Dongli Hospital from January 2020 to December 2022 were selected and divided into mild,moderate and severe cases,and were given antiviral treatment with Aciclovir,Famciclovir,Valaciclovir and Ganciclovir;Vitamin B1,mecobalamin,and neurotropin nourished nerves;Nimesulide,Pregabalin,Parecxib for pain relief.By using the method of integrated traditional Chinese and Western medicine,differentiation and treatment were carried out.Assisted by cupping,external application of traditional Chinese medicine,red light,and helium neon laser treatment.Results After 7 days of treatment with integrated traditional Chinese and Western medicine,the patients'blisters in the affected areas dried up and scabbed.The pain in 20 patients was significantly relieved.The pain disappeared in 16 patients 10 days later.The pain in 2 patients with diabetes mellitus lasted for 1 month and then gradually recovered.Conclusion The integrated traditional Chinese medicine and Western medicine was effective in the treatment of shingles in the head and face,and could significantly reduce the incidence of neuralgia,which was worthy of clinical promotion and application.
