彩色多普勒超声对颈动脉硬化斑块溃疡诊断价值

摘　　要：目的探讨彩色多普勒超声对颈动脉硬化斑块溃疡的诊断价值,为颈动脉硬化斑块溃疡患者的早期准确诊断提供理论和实践依据。方法选取2018年6月—2020年6月梅州市第二中医医院收治的疑似颈动脉硬化斑块患者80例,使用高分辨率彩色多普勒超声进行检查,对斑块部位、分型特征进行统计,并着重分析溃疡型斑块的声像学特点。结果80例患者共检出颈动脉粥样硬化斑块113个。113个硬化斑块中,软斑36例,占31.86%,斑块较为明显,呈低中度回声;硬斑41例,占36.28%,表面光滑,且有强回声伴声影;扁平斑16例,占14.16%,呈局部隆起状,均匀低回声;溃疡斑20例,占17.70%,表面不平整,中央有凹陷,回声强弱不均匀。溃疡斑的声像学特点为斑块沉积于动脉内膜下层,形态不规则,弥散分布,内膜回声不均匀,且不连续,斑块内可见低回声区,部分典型病灶可见顶部的“火山口”征,并有血流进入溃疡斑内部。结论高分辨率的彩色多普勒超声能够对颈动脉粥样硬化斑块的分布位置和分型进行较好的显示,可为临床诊断和治疗提供依据。Objective To explore the diagnostic value of color Doppler ultrasound in carotid atherosclerotic plaque ulcer,and provide theoretical and practical basis for the early and accurate diagnosis of carotid atherosclerotic plaque ulcer.Methods A total of 80 patients with suspected carotid atherosclerotic plaques admitted to Meizhou Second Hospital of Traditional Chinese Medicine from June 2018 to June 2020 were selected for examination with high-resolution color Doppler ultrasound,and the plaque location and classification characteristics were statistically analyzed,and the sonographic characteristics of ulcerative plaques were emphatically analyzed.Results A total of 113 carotid atherosclerotic plaques were detected in 80 patients.Of the 113 sclerotic plaques,there were 36 soft plaques,accounting for 31.86%,the plaques were more obvious and showed low and moderate echo;there were 41 cases of hard spots,accounting for 36.28%,with smooth surface and strong echo with sound shadow;16 cases had flat spots,accounting for 14.16%,which were locally raised and uniformly hypoechoic;20 cases had ulcerative spots,accounting for 17.70%,with uneven surface,depression in the center and uneven echo intensity.The ultrasonographic characteristics of ulcer plaque were that the plaque was deposited in the lower intimal layer of the artery,with irregular shape,diffuse distribution,uneven and discontinuous intimal echo,low echo area could be seen in the plaque,and"crater"sign at the top could be seen in some typical lesions,and blood flow entered the ulcer plaque.Conclusion High resolution color Doppler ultrasound could better display the distribution and classification of carotid atherosclerotic plaques,which could provide the basis for clinical diagnosis and treatment.
