摘　　要：目的通过临床药师对患者的用药监护,促进精神药物的合理应用。方法临床药师通过熟练掌握丙戊酸钠及精神药物的药理作用、药物不良反应以及采取的防范措施,对1例丙戊酸钠所致横纹肌溶解伴肝损伤的患者进行药学监护。结果临床药师协助临床医生共同制定方案,停用致横纹肌溶解的药物丙戊酸钠,并给予支持治疗,避免发生急性肾损伤。治疗后患者肝肾指标均恢复至正常水平。结论临床药师及时辨别药源性疾病,提高临床医生对药物引起横纹肌溶解的认识,保证了患者的用药安全,同时为临床安全有效地使用丙戊酸钠提供参考。Objective To promote the rational use of psychotropic drugs through the clinical pharmacist′s drug monitoring of patients.Methods The clinical pharmacist,through mastering the pharmacological action of sodium valproate and other psychotropic drugs,adverse drug reactions and preventive measures,carried out pharmaceutical monitoring on a patient with rhabdomyolysis and liver injury caused by sodium valproate.Results Clinical pharmacists assisted clinician to develop the treatment plan,discontinue the rhabdomyolytic drug of sodium valproate and provide supportive treatment to avoid acute kidney injury.After treatment,the liver and kidney indicators all returned to normal levels.Conclusion Clinical pharmacists can promptly identify drug-induced diseases to improve the understanding of drug-induced rhabdomyolysis of clinician,to ensure the medication safety and provide references for the effective and safe use of sodium valproate clinically.
