摘　　要：目的:探讨可吸收血管封合医用胶在单侧胸廓内动脉冠状动脉旁路移植术(CABG)中的止血效果及对桥血管即时流量的影响。方法:选取冠状动脉粥样硬化性心脏病病人60例作为研究对象,择期在全身麻醉体外循环下行应用单侧胸廓内CABG。根据术中是否使用可吸收血管封合医用胶,将病人分为观察组和对照组,各30例。比较观察组病人术中使用可吸收血管封合医用胶前后桥血管即时血流参数;比较2组病人的围手术期相关指标。结果:观察组病人术中使用可吸收血管封合医用胶前后平均血流量、搏动指数差异均无统计学意义(P>0.05)。观察组病人手术时间及术后24、48 h引流量均少于对照组(P<0.05~P<0.01)。结论:可吸收血管封合医用胶应用于单侧胸廓内动脉的CABG中,对桥血管的即时血流没有影响,可有效止血,减少术后引流量,缩短手术时间。Objective:To investigate the hemostatic effect of absorbable medical vascular sealant in coronary artery bypass grafting of unilateral internal thoracic artery and its influence on the transit-time flow of the bridge vessel.Methods:Sixty patients with coronary atherosclerosis heart disease were selected as study subjects,who were scheduled with coronary artery bypass grafting of unilateral internal thoracic artery in cardiopulmonary bypass under general anesthesia.The patients were divided into observation group and control group according to whether absorbable medical vascular sealant was used during surgery,with 30 cases in each group.The transit-time blood flow parameters of the bridge vessel of patients during surgery before and after the use of absorbable medical vascular sealant in the observation group were compared,and related indicators during perioperative period of patients in the two groups were compared.Results:There were no statistically significant differences in average blood flow and pulsatility index of patients in the observation group before and after the use of absorbable medical vascular sealant during surgery(P>0.05).The operation time,postoperative 24-hour drainage volume and postoperative 48-hour drainage volume of patients in the observation group were lower than those in the control group(P<0.05 to P<0.01).Conclusions:The absorbable medical vascular sealant has no effect on the transit-time blood flow of the bridge vessel during coronary artery bypass grafting of unilateral internal thoracic artery,and can effectively stop bleeding,reduce postoperative drainage volume and shorten the operation time.
