福清地区3TC+TDF+EFV方案对HIV患者血脂水平影响的研究

摘　　要：目的 研究拉米夫定、替诺福韦、依非韦仑(3TC+TDF+EFV)抗病毒方案对HIV患者血脂水平的影响。方法 回顾性分析截止2022年12月在我院接受3TC+TDF+EFV抗病毒治疗满1年以上184例HIV患者的临床资料,比较服用抗病毒药物前、服药6月、1年时的血脂水平变化。97例患者服药时间满3年以上,分析抗病毒方案与血脂水平变化之间是否存在一定相关性。分析服药1年以上患者血脂异常的发生情况。统计分析采用SPSS 22.0。结果 184例患者治疗6月、1年时总胆固醇(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)呈进行性升高趋势。97例服药满3年以上的患者,TG、LDL呈逐步上升趋势。和治疗1年时相比,治疗2年时、3年时LDL数值上升,差异有统计学意义。高效抗反转录病毒治疗(HAART)3年时分别有34.0%、22.7%、17.5%患者的TG、TC、LDL数值高于正常值上限。HAART治疗1年时,TG、TC、LDL数值超出正常值的患者比例显著升高。结论 接受该方案治疗后早期对患者血脂水平的影响较为明显,随着抗病毒时间延长,仅对LDL的升高有影响。OBJECTIVE To analyze the effect of lamivudine,tenofovir and efviren antiviral regimen on blood lipid level in HIV patients.METHODS A retrospective cohort study was conducted of 184 HIV patients who had received antiviral treatment of lamivudine+tenofovir+efviren(3TC+TDF+EFV)in our hospital for more than 1 year and had complete follow-up clinical data by December 2022.Compare the changes of blood lipid level before receiving antiviral drugs,6 months and 1 year after receiving drugs among these patients.Analyzed whether there was a correlation between 3TC+TDF+EFV regimen and blood lipid level in 97 patients who had received antiretroviral treatment(ART)more than 3 years.Analyze the occurrence of dyslipidemia in patients receiving medicine for more than 1 year.SPSS 22.0 statistical software was used for statistical analysis.RESULTS The total cholesterol,high-density lipoprotein and low-density lipoprotein in 184 patients showed a progressive increase at 6 months,1 year after receiving the treatment with high efficiency antiretroviral therapy(HAART),and the difference was statistically significant compared with the baseline value.The total cholesterol and low density lipoprotein showed a gradually increasing trends of 97 patients who had took medicine for more than 3 years.The value of low density lipoprotein had a statistically significant increasing after 2 and 3 years of treatment compared with 1 year of treatment.At 3 years of treatment,triglyceride,total cholesterol and low density lipoprotein values of 34.0%,22.7%and 17.5%patients were higher than the upper limit of normal values,respectively.After 1 year of HAART treatment,the proportion of patients with values of triglyceride,total cholesterol and low density lipoprotein above normal was increased,and the difference was statistically significant compared with baseline.CONCLUSION After receiving 3TC+TDF+EFV regimen,the effect of early treatment on blood lipid level of patients is more obvious,and the occurrence of dyslipidemia is relatively common.However,with
