摘　　要：目的探讨腹部带蒂皮瓣和V-Y推进皮瓣在修复手外伤软组织缺损临床实践中的疗效差异。方法选取2020年1月-2022年1月临床收治的手部外伤软组织缺损患者60例为研究对象,按照手术方式不同分成观察组和对照组,各30例。对照组给予腹部带蒂皮瓣术治疗,观察组给予V-Y推进皮瓣术治疗。术后对两组患者缺损处血流速度(BFV)和血流灌注量相对值(BPR)、创面愈合时间、住院时间、临床疗效和并发症发生情况进行观察和比较。结果观察组的BFV和BPR血流指标优于对照组,创面愈合时间早于对照组,住院时间少于对照组,治疗优良率高于对照组,并发症发生率低于对照组,差异均具有统计学意义(P<0.05)。结论与腹部带蒂皮瓣术相比,采用V-Y推进皮瓣术修复手部外伤软组织缺损,能够改善患指局部血运情况,促进患者恢复,提高临床疗效,并有效控制并发症发生,是临床用于修复手部外伤软组织缺损的可靠方案之一。Objective To compare the clinical effects of abdominal pedicle flap and V-Y advancement flap in repairing hand traumatic soft tissue defects.Methods From January 2020 to January 2022,60 patients with hand trauma and soft tissue defects who were treated in our hospital were selected as the study objects.They were divided into study group and control group according to different surgical methods,with 30 patients in each group.The control group was treated with abdominal pedicle skin flap,while the study group was treated with V-Y advancement skin flap.The blood flow at the defect site blood flow velocity(BFV)and relative value of blood flow perfusion(BPR),wound healing time,hospital stay,clinical efficacy and complications of the two groups were observed and compared.Results After operation,the BFV and BPR blood flow indexes of the study group were better than those of the control group,the wound healing time was earlier than that of the control group,the hospital stay was less than that of the control group,the excellent and good rate of treatment was higher than that of the control group,and the incidence of complications was lower than that of the control group,and the differences were significant(P<0.05).Conclusion Compared with abdominal pedicled skin flap,V-Y advancement flap can improve the local blood circulation of patients,promote the recovery of patients,improve the clinical efficacy,and effectively control the occurrence of complications.It is one of the reliable schemes for repairing hand traumatic soft tissue defects.
