应用自制克氏针外固定架治疗虎口挛缩

摘　　要：目的探讨应用自制克氏针外固定架治疗虎口挛缩的手术方法及疗效。方法2015年8月-2021年7月,使用自制克氏针外固定架治疗虎口挛缩20例,均有完整随访资料。采用臂丛神经阻滞麻醉,桡动脉鼻烟窝穿支皮瓣等方法修复虎口缺损后,在第1、2掌骨颈附近打入直径1.5 mm克氏针,并自制克氏针横梁,撑开虎口,弯曲克氏针端固定,依需要上下滑动横梁,调节虎口开大角度,固定4~6周后拆除。结果20例均获随访,随访时间4个月。拇指活动范围均明显增加,功能明显改善。结论使用自制克氏针外固定架配合皮瓣等方案治疗虎口挛缩,方法简单,经济实惠,治疗效果满意。Objective To investigate the efficacy of the surgical method by using self-designed K-wire external fixator to treat the first web space contracture.Methods Twenty cases who had the first web space contracture were treated by using a self-designed K-wire external fixator from August 2015 to July 2021.20 cases had complete follow-up data.We transplanted snuff box flap to cover the first web space vacancy after brachial plexus block anesthesia,then Drilled two K-wire in the vicinity of the first and second metacarpal neck.We Distracted web space by using Self-designed K-wire beam and bend off them to mount up.So we could slide the beam up or down according to the first web space open wide angle.Removed the system after 4 weeks.Results Twenty patients were followed up for 4 months.Thumb significantly increased the scope of activities,function improved significantly.Conclusion It is simple and affordable that Using self-designed K-wire external fixator when we treat the first web space contracture with flap,and we can got satisfactory results too.
