掌侧入路钢板内固定与腕关节外固定支架治疗桡骨远端C型骨折的疗效比较

摘　　要：目的探究掌侧入路钢板内固定与腕关节外固定支架治疗桡骨远端C型骨折的临床疗效。方法选取2019年7月-2022年6月收治的100例桡骨远端C型骨折患者为主要研究对象,采用随机数字表法分为两组,其中一组采用腕关节外固定支架治疗,另一组采用掌侧入路钢板内固定治疗,比较两组临床指标、术后骨折愈合情况、Cooney腕关节评分及并发症发生率。结果外固定组手术时间、术中出血量、住院时间、骨折愈合时间均低于内固定组,差异具有统计学意义(P<0.05)。两组掌倾角、尺偏角、桡骨高度及Cooney腕关节评分、并发症发生率比较,差异均无统计学意义(P>0.05)。结论腕关节外固定支架、掌侧入路钢板内固定在治疗桡骨远端C型骨折方面,均具有良的临床应用价值,但是腕关节外固定支架能缩短患者手术时间、术中出血量、住院时间及术后愈合时间,而掌侧入路钢板内固定有利于患者尽早展开腕功能训练。Objective To investigate the clinical efficacy of palmar approach plate internal fixation and external fixation of wrist joint in the treatment of distal radius C-type fracture.Methods One hundred patients with C-type fracture of the distal radius admitted in our hospital from July 2019 to June 2022 were selected as the main research objects.They were randomly divided into two groups by using the method of random number table.One group was treated with external fixation of wrist joint,and the other group was treated with internal fixation of volar plate.The clinical indicators,fracture healing,Cooney wrist score and complication rate of two groups were compared.Results The operation time,intraoperative blood loss,hospitalization time and fracture healing time of patients in the external fixation group were lower than those in the internal fixation group(P<0.05).There was no significant difference between two groups in palm angle,ulnar deflection angle,radius height,Cooney wrist score and complication rate(P>0.05).Conclusion Wrist external fixator and volar approach plate internal fixation have good clinical application value in the treatment of distal radius C-type fracture,but wrist external fixator can shorten the operation time,intraoperative blood loss,hospital stay and postoperative healing time of patients,while volar approach plate internal fixation is conducive to early wrist functional exercise of patients.
