摘　　要：目的比较小切口直视下保留支架与腹腔镜下不保留支架在输卵管吻合术中的应用效果。方法选取2017年1月至2022年1月收治的60例输卵管结扎术后行输卵管吻合术的患者为研究对象,根据手术方法不同将其分为对照组(28例)和研究组(32例)。对照组实施腹腔镜下不保留支架的输卵管吻合术,研究组实施小切口直视下保留支架的输卵管吻合术。比较两组的手术时间、术中出血量、输卵管复通率、宫内妊娠成功率及异位妊娠率。结果研究组的手术时间短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。研究组的输卵管复通率、宫内妊娠成功率高于对照组,差异具有统计学意义(P<0.05);两组的异位妊娠率比较,差异无统计学意义(P>0.05)。结论小切口直视下保留支架输卵管吻合术可缩短手术时间,减少术中出血量,提高输卵管复通率及宫内妊娠成功率,且异位妊娠发生率低,值得临床推广及应用。Objective To compare the application effect of retaining stent under small incision direct vision and laparoscopic non-retaining stent in fallopian tube anastomosis.Methods Sixty patients who underwent fallopian tube anastomosis after tubal ligation from January 2017 to January 2022 were selected as the study subjects.According to different surgical methods,the patients were divided into control group(28 cases)and study group(32 cases).The control group underwent laparoscopic non-retaining stent in fallopian tube anastomosis,and the study group underwent retaining stent under small incision direct vision in fallopian tube anastomosis.The operation time,intraoperative blood loss,fallopian tube recanalization rate,intrauterine pregnancy success rate and ectopic pregnancy rate were compared between the two groups.Results The operation time of the study group was shorter than that of the control group,and the intraoperative blood loss was less than that of the control group,the differences were statistically significant(P<0.05).The fallopian tube recanalization rate and intrauterine pregnancy success rate of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05);there was no significant difference in ectopic pregnancy rate between the two groups(P>0.05).Conclusion Retaining stent under small incision direct vision in fallopian tube anastomosis can shorten the operation time,reduce the amount of bleeding during operation,improve the fallopian tube recanalization rate and the success rate of intrauterine pregnancy,and the incidence of ectopic pregnancy is low,which is worthy of clinical promotion and application.
