摘 要:目的探讨益心附葶饮对心衰大鼠心肌能量代谢的影响及作用机制。方法本实验于2021年11月将购自成都达硕实验动物有限公司的28只Wistar大鼠随机分为空白组、模型组、西药组、中药组。除了空白组,其余各组建立异丙肾上腺素诱导的心衰大鼠模型,造模后中药组给予益心附葶饮灌胃,西药组给予左卡尼汀口服液灌胃,空白组和模型组给予等剂量无菌生理盐水灌胃。4周后,采集各组大鼠心肌组织样本,观察游离脂肪酸(FFA)及5'单磷酸腺苷活化蛋白激酶(AMPK)信号通路中基因乙酰辅酶A羧化酶(ACC)、AMPK表达水平,分析心肌组织病理及电镜检查结果。结果西药组的FFA水平显著低于模型组(P<0.01);中药组的FFA水平低于模型组(P<0.05)。西药组与中药组的AMPK m RNA相对表达量显著高于模型组(P<0.01)。病理结果示中药组较模型组心肌中膜层心肌纤维增生及炎性细胞浸润有所改善;西药组较中药组膜层心肌纤维坏死明显。透射电镜示西药组、中药组线粒体结构均改善。结论益心附葶饮可改善心肌能量代谢重构,其作用机制可能与修复线粒体、改变AMPK信号通路中基因ACC、AMPK表达水平,降低心肌FFA水平有关。Objective To investigate the effect and mechanism of Yixin Futing decoction on myocardial energy metabolism in rats with heart failure.Methods In this experiment,28 Wistar rats purchased from Chengdu Dashuo Experimental Animal Co.,Ltd.were randomly divided into blank group,model group,western medicine group and Chinese medicine group in November 2021.In addition to the blank group,isoproterenol-induced heart failure rat model was established in the other groups.After the model was established,the Chinese medicine group was given Yixin Futing decoction by gavage,the western medicine group was given L-carnitine oral liquid by gavage,and the blank group and the model group were given equal doses of sterile normal saline by gavage.After 4 weeks,the myocardial tissue samples of rats in each group were collected to observe the expression levels of free fatty acid(FFA)and genes acetyl-CoA carboxylase(ACC)and AMPK in adenosine monophosphate activated protein kinase(AMPK)signaling pathways,and the results of myocardial histology and electron microscopy were analyzed.Results The level of FFA in the western medicine group was significantly lower than that in the model group(P<0.01);the level of FFA in the Chinese medicine group was lower than that in the model group(P<0.05).The relative expression of AMPK mRNA in the western medicine group and the Chinese medicine group was significantly higher than that in the model group(P<0.01).The pathological results showed that the membrane myocardial fiber hyperplasia and inflammatory cell infiltration in the myocardium improved in the Chinese medicine group compared with the model group;the membrane myocardial fiber necrosis was more obvious in the western medicine group compared with the Chinese medicine group.Transmission electron microscopy showed that mitochondrial structure improved in both the western medicine group and the Chinese medicine group.Conclusion Yixin Futing decoction can improve myocardial energy metabolism remodeling,and its mechanism of action may be related to
