高位结扎联合点式剥脱术治疗大隐静脉曲张患者的效果

摘　　要：目的:观察高位结扎联合点式剥脱术治疗大隐静脉曲张患者的效果。方法:回顾性分析2020年1月至2022年12月该院收治的70例大隐静脉曲张患者的临床资料,按照手术方式不同将其分为对照组和观察组各35例。对照组行高位结扎联合传统剥脱术治疗,观察组行高位结扎联合点式剥脱术治疗。比较两组临床疗效,手术相关指标(术中出血量、手术时间、住院时间)水平,手术前后炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]水平,凝血功能指标[纤维蛋白原(FIB)、D-二聚体(D-D)、凝血酶时间(TT)、凝血酶原时间(PT)]水平,并发症发生率,以及复发率。结果:观察组治疗总有效率为97.14%(34/35),高于对照组的71.43%(25/35),差异有统计学意义(P<0.05);观察组术中出血量少于对照组,手术时间、住院时间均短于对照组,差异有统计学意义(P<0.05);术后3 d,观察组TNF-α、IL-6水平均低于对照组,差异有统计学意义(P<0.05);术后,观察组TT、PT、D-D水平均低于对照组,FIB水平高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为5.71%(2/35),低于对照组的25.71%(9/35),差异有统计学意义(P<0.05);两组复发率比较,差异无统计学意义(P>0.05)。结论:高位结扎联合点式剥脱术治疗大隐静脉曲张患者可提高治疗总有效率,改善手术相关指标和凝血功能指标水平,减轻术后炎症反应,降低并发症发生率,效果优于高位结扎联合传统剥脱术治疗。Objective:To observe effects of high ligation combined with point stripping in treatment of patients with varicose great saphenous veins.Methods:The clinical data of 70 patients with varicose great saphenous veins admitted to the hospital from January 2020 to December 2022 were retrospectively analyzed.According to different surgical methods,they were divided into control group and observation group,35 cases in each group.The control group was treated with high ligation combined with traditional stripping,while the observation group was treated with high ligation combined with point stripping.The clinical efficacy,the operation-related indicator levels(intraoperative blood loss,operation time,hospitalization time),the inflammatory factor levels[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]before and after the surgery,the coagulation function indicator levels[fibrinogen(FIB),D-dimer(D-D),thrombin time(TT),prothrombin time(PT)],the complication rate,and the recurrence rate were compared between the two groups.Results:The total effective rate of treatment in the observation group was 97.14%(34/35),which was higher than 71.43%(25/35)in the control group,and the difference was statistically significant(P<0.05).The intraoperative blood loss in the observation group was less than that in the control group,the operation time and the hospitalization time were shorter than those in the control group,and the differences were statistically significant(P<0.05).At 3 days after the surgery,the levels of TNF-αand IL-6 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the surgery,the levels of TT,PT and D-D in the observation group were lower than those in the control group,the level of FIB was higher than that in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 5.71%(2/35),which was lower than 25.71%(9/35)in the control group,and the difference was sta
