摘 要:控制术中出血是肝脏手术的核心技术,也是提高患者手术获益、降低手术风险的重要方式。近年来,临床上对中心静脉压(central venous pressure,CVP)这一传统静态指标在肝切除术中的应用价值产生诸多质疑。每搏量变异度(stroke volume variation,SVV)作为监测血流动力学的动态指标,在评估患者容量反应性及指导补液中发挥着重要作用,其具有检测创伤小、敏感性高、无需人为校正等特点,目前已在临床中得到广泛应用。本文将从SVV的基本原理及容量反应性、SVV与肝切除术效果的关系、SVV的影响因素等方面进行综述,旨在为临床工作提供参考。Control of intraoperative bleeding is the core technology of liver surgery,and it is also an important way to improve the benefit of surgery and reduce the risk of surgery.In recent years,there have been many clinical doubts about the application value of the traditional static indicator of central venous pressure(CVP)in hepatectomy.stroke volume variation(SVV),as a dynamic indicator for monitoring hemodynamics,plays an important role in evaluating patients’volumetric reactivity and guiding fluid rehydration.It has the characteristics of low detection trauma,high sensitivity and no need for manual correction,and has been widely used in clinical practice.This article will review the basic principle and volume reactivity of SVV,the relationship between SVV and the effect of hepatectomy,and the influencing factors of SVV,in order to provide referen ce for clinical work.
