基于数据挖掘的古代针灸治疗面瘫选穴及配伍规律分析

摘　　要：目的:挖掘中医典籍中古代医家针灸治疗面瘫的选穴及配伍规律,为现代针灸治疗面瘫提供借鉴。方法:检索《中华医典》(第五版)中针灸治疗面瘫的腧穴处方并建立数据库,将其录入中医传承辅助平台系统,运用软件进行数据挖掘分析。结果:共纳入符合要求的腧穴处方201条,涉及43本古籍,其中单穴处方111条,多穴配伍处方90条,共包含45个穴位;应用频次较高的腧穴由高到低依次为:地仓、颊车、水沟、合谷、承浆、听会等;最常见的腧穴组合按频次由高到低依次为“地仓,颊车”“合谷,颊车”“地仓,合谷”“地仓,水沟”“地仓,听会”等;置信度为1的腧穴组合有“颊车,听会→地仓”“颊车,水沟→合谷”“地仓,颊车,水沟→合谷”。设置相关度为8、惩罚度为2,得到核心穴组6组和新处方3首。结论:古代医家针灸治疗面瘫具有精选近部穴位、注重远近配穴,多用阳经穴、尤重阳明经,注重特定穴,善用多经交会穴的选穴规律,该选穴及配伍规律和新穴位处方为临床医师制定方案提供客观依据。Objective:To explore the rules of acupoint selection and compatibility of acupuncture and moxibustion in the treatment of facial paralysis by ancient doctors in traditional Chinese medicine books,and to provide reference for the treatment of facial paralysis by modern acupuncture and moxibustion.Methods:The acupoint prescriptions of acupuncture and moxibustion in the treatment of facial paralysis in the fifth edition of Chinese Medical Dictionary were retrieved and the database was established,which was entered into the TCM inheritance auxiliary platform system,and the software was used for data mining and analysis.Results:A total of 201 acupoint prescriptions were included,involving 43 ancient books,including 111 single-point prescriptions and 90 multi-point prescriptions,including 45 acupoints.The most frequently applied acupoints from high to low were:Dicang,Jiacha,Shuigou,Hegu,Chengjiang,Tinghui,etc.The most common acupoint combinations,in order of frequency from high to low,were"Dicang,Jiacha","Hegu,Jiacha","Dicang,Hegu","Dicang,Shuigou","Dicang,Tinghui",etc.The acupoint combinations with a confidence level of 1 were"Jiacha,Tinghui→Dicang","Jiacha,Shuigou→Hegu","Dicang,Jiacha,Shuigou→Hegu".By setting the correlation degree as 8 and punishment degree as 2,six core acupoint groups and three new prescriptions were obtained.Conclusion:The acupoint selection rules of an‐cient physicians in the treatment of facial paralysis include selecting proximal acupoints,paying attention to matching acupoints near and far,using Yang meridian acupoints,especially Yang Ming meridian,paying attention to specific acupoints,and making good use of intersection acupoints.The acupoint selection and compatibility rules and new acupoint prescription provide objective basis for clinicians to make plans.
