摘　　要：要综述肩关节置换相关肱骨假体周围骨折的研究进展,提出存在问题并展望研究方向。广泛查阅近年国内外肩关节置换相关肱骨假体周围骨折的相关文献,对肩关节置换相关肱骨假体周围骨折的流行病学、危险因素、分类、治疗方法、康复和预防进行总结。随着人口年龄的增长、运动量的增加、肩关节置换手术的普及和翻修手术的增多,肩关节置换相关肱骨假体周围骨折的发生率呈逐年上升的趋势。肱骨假体周围骨折的治疗目的是在保持假体稳定的同时实现骨折愈合,保留肱盂关节运动,恢复肩关节功能。治疗策略从非手术治疗、切开复位内固定到关节置换翻修不一而足。最佳治疗方法的选择,取决于骨折类型、位置、骨折时间(术中与术后比较)、假体的稳定性、患者的年龄、性别、骨质量、活动水平和并存疾病。肩关节置换相关肱骨假体周围骨折多进行手术治疗,手术方法多样,为提高手术效果、减少术后并发症发生,需结合患者的自身状况和骨折类型选择最合适的治疗策略。Objective:To review research progress in periprosthetic humeral fracture related with shoulder arthroplasty,put forward the existing problems and prospect the research direction.Methods:The domestic and foreign literature about periprosthetic humeral fracture related with shoulder arthroplasty in recent years was extensively consulted,and the epidemiology,risk factors,classification,treatment methods,rehabilitation and prevention of periprosthetic humeral fractures related to shoulder arthroplasty were summarized.Results:The incidence of periprosthetic fractures of humeral related with shoulder arthroplasty is increasing year by year with the increase of population age,physical activity,popularity of shoulder arthroplasty and revision surgery.The objective of the treatment of periprosthetic fractures of the humeral is to achieve fracture healing,preserve motion of the glenoid joint,and restore shoulder function while maintaining stability of the prosthesis.Treatment strategies range from nonoperative treatment,open reduction and internal fixation to joint replacement and revision.The choice of the best treatment depends on fracture type,location,fracture duration(intraoperative versus postoperative),prosthesis stability,patients age,sex,bone quality,activity level,and comorbidities.Conclusion:The periprosthetic fractures of humeral related with shoulder arthroplasty are often treated surgically,with various surgical methods.In order to improve the surgical effect and reduce postoperative complications,it is necessary to select the most appropriate treatment strategy based on the patients own condition and fracture type.
