中医药治疗烧烫伤患者临床效果的荟萃分析

摘　　要：目的评价中医药治疗烧烫伤的临床效果。方法检索Cochrane、PubMed、Embase、Ovid、CNKI、Wanfang数据库建库至2023年4月18日期间的中医药治疗烧烫伤临床随机对照研究(RCT)进行系统评价。结果共纳入31篇RCT进行荟萃分析。中医药治疗可提高烧烫伤患者的临床总有效率(RR=1.20,95%CI:1.15~1.26,P<0.001)、细菌清除率(RR=1.68,95%CI:1.44~1.97,P<0.001)和创面愈合率(SMD=1.37,95%CI:0.81~1.93,P<0.001),降低瘢痕发生率(RR=0.29,95%CI:0.23~0.38,P<0.001)和疼痛评分(SMD=-1.76,95%CI:-2.40~-1.11,P<0.001),缩短创面愈合时间(SMD=-1.69,95%CI:-2.06~-1.32,P<0.001),且无明显不良反应事件发生。结论中医药治疗烧烫伤,可显著提高患者临床总有效率、细菌清除率、创面愈合率,缩短创面愈合时间,降低瘢痕发生率及疼痛评分。中医药制剂治疗烧烫伤具有良好的应用前景,但其远期疗效及安全性有待开展更多高质量、大样本、长期随访的RCT加以验证。Objective To evaluate the clinical effect of traditional Chinese medicine in treating burns.Methods Randomized controlled trials(RCTs)studies on treatment of burns with traditional Chinese medicine were searched from Cochrane,PubMed,Embase,Ovid,CNKI and Wanfang databases for systematic review.The retrieval time limit was from the establishment of the databases to April 18,2023.Results A total of 31 RCTs were included in the meta-analysis.Traditional Chinese medicine treatment could improve the clinical effective rate(RR=1.20,95%CI:1.15-1.26,P<0.001)bacterial clearance rate(RR=1.68,95%CI:1.44-1.97,P<0.001)and wound healing rate(SMD=1.37,95%CI:0.81-1.93,P<0.001),reduce the incidence of scar(RR=0.29,95%CI:0.23-0.38,P<0.001)and pain scores(SMD=-1.76,95%CI:-2.40--1.11,P<0.001),and shorten the wound healing time(SMD=-1.69,95%CI:-2.06--1.32,P<0.001),and no obvious adverse reactions occurred.Conclusion Traditional Chinese medicine treatment of burns can significantly improve the clinical effective rate,bacterial clearance rate and wound healing rate,shorten wound healing time,reduce the incidence of scar and pain score.Traditional Chinese medicine preparations have good application prospects,but the long-term efficacy and safety need to be verified by more RCTs with high quality,large samples and long-term follow-up.
