摘　　要：目的分析子宫下段剖宫产分娩患者住院费用的构成及其影响因素,为合理控制住院费用提供依据。方法收集广州市某医院2019年1月1日-2020年12月31日行子宫下段剖宫产术分娩住院患者2565例,利用单因素和多重线性回归方法对住院费用的影响因素进行分析。结果子宫下段剖宫产分娩患者住院费用构成前5位分别为材料费(21.3%)、药费(19.0%)、手术费(16.1%)、治疗费(11.1%)和麻醉费(10.2%)。住院天数、年龄、抢救、病理检查、输血、病例分型、付费方式是住院费用的主要影响因素。结论子宫下段剖宫产分娩患者的住院费用较高,费用构成不合理,要合理控制材料费和药费。住院天数是影响住院费用最重要的因素,实施临床路径缩短住院天数是控制住院费用的重要措施。Objectives This study aims to analyze the hospitalization cost composition and its influencing factors of patients with lower uterine cesarean section,so as to provide a basis for reasonable control of hospitalization expenses.Methods 2565 inpatients who underwent lower uterine cesarean section in a hospital in Guangzhou from January 1,2019 to December 31,2020 were collected.The single factor and multiple linear regression methods were used to analyze the influencing factors of hospitalization expenses.Results The top five hospitalization expenses of patients with lower cesarean section were material(21.3%),medicine(19.0%),surgical(16.1%),treatment(11.1%),and anesthesia(10.2%).Length of stay,age,rescue,pathological examination,blood transfusion,case classification,and payment method were the main influencing factors of hospitalization expenses.Conclusions The hospitalization cost of patients with lower uterine cesarean section is high and the cost composition is unreasonable.It is necessary to reasonably control material costs and drug costs.The most important influencing factor of the hospitalization cost is the length of stay.The implementation of a clinical pathway to shorten hospitalization days is an important measure to control the hospitalization cost.
