山西省三级医院住院死亡病例统计分析

摘　　要：目的分析山西省三级医院2018-2021年住院死亡病例年龄、性别、死因分布情况,为省级卫生行政部门确定卫生工作重点及合理配置医疗资源提供决策依据。方法收集山西省46所三级医院2018年1月1日-2021年12月31日22598例住院死亡病例,根据ICD-10国际疾病分类原则采用Excel 2016对性别、年龄、死因构成方面进行统计分析。结果全省三级医住院患者总人数为6668075例,死亡患者共22598例,病死率0.34%,2020年死亡人数最高,病死率也达到峰值。性别构成上男性病死率为0.46%,女性病死率为0.22%,男性患者病死率高于女性患者。不同年龄段患者病死率差异较大,病死率最高的是80岁及以上人群组(1.79%),其次是70~80岁人群组(0.59%)。在死因分布及构成中死因前3位分别是循环系统疾病、恶性肿瘤和呼吸系统疾病。结论2018-2021年全省病死率最高的是80岁以上人群,男性死亡率高于女性,循环系统疾病是是造成死亡的主要原因。相关部门应合理配置医疗资源,加强对肿瘤、循环系统疾病和呼吸系统疾病的学科建设,以降低患者的病死率。Objectives This study aims to analyze the distribution of age,sex,and cause of death of inpatients in tertiary hospitals in Shanxi Province,so as to provide decision-making basis for provincial health administrative departments to determine the focus of health work and rationally allocate medical resources.Methods 22598 inpatient death cases were collected from 46 tertiary hospitals in Shanxi Province from January 1,2018 to December 31,2021.According to the ICD-10 international disease classification principle,Excel 2016 was used for statistical analysis of gender,age,and cause of death.Results The total number of inpatients in tertiary hospitals in the province was 6668075,and 22598 patients died,with a fatality rate of 0.34%.In 2020,the number of deaths was the highest,and the fatality rate also reached a peak.In terms of gender composition,the case fatality rate of male patients was 0.46%,and that of female patients was 0.22%.The case fatality rate of male patients was significantly higher than that of female patients.The case fatality rate of patients in different age groups varied greatly.The highest case fatality rate is in the group aged 80 and above(1.79%),followed by the group aged 70-80(0.59%).In the distribution and composition of causes of death,the top three causes of death were circulatory system diseases,malignant tumors,and respiratory system diseases.Conclusions From 2018 to 2021,the highest case fatality rate in the province was among people over 80 years old.The male mortality rate was higher than the female mortality rate.The circulatory system disease was the main cause of death.Relevant departments should reasonably allocate medical resources and strengthen the discipline construction of tumors,circulatory diseases,and respiratory diseases to reduce the mortality of patients.
