腹腔镜微创治疗与传统开腹治疗对肝胆结石患者围术期指标的影响

摘　　要：目的 探究腹腔镜微创治疗、传统开腹手术对于肝胆结石患者围术期各项指标的影响。方法 将本院2021年7月-2022年7月收治的84例肝胆结石患者纳入研究,令患者自愿选择手术方式,分为开腹治疗组(n=42;传统开腹手术)、微创组(n=42;腹腔镜微创治疗),对比两组患者围术期各项指标变化。结果 微创组各项手术指标(手术时长、术中出血量、排气时间、首次下床活动时间、拔管时间、住院时间)显著优于开腹治疗组(P<0.05);术后1-3d VAS评分显著低于开腹治疗组(P<0.05);并发症发生率显著低于开腹治疗组(P<0.05);治疗总有效率显著高于开腹治疗组(P<0.05)。结论 与传统开腹治疗相比,腹腔镜微创治疗效果更为显著,具有创伤小、恢复时间短、出血量少等优势,同时还可降低患者围术期痛感,减少围术期并发症的发生。Objective To explore effect of laparoscopic minimally invasive treatment and traditional open surgery quality on various perioperative indicators of hepatolithiasis patients.Methods The paper chose 84 cases hepatolithiasis patients admitted to our hospital from July 2021 to July 2022,with surgical methods chosen voluntarily,and divided them into open surgery group(n=42;traditional open surgery)and minimally invasive group(n=42;laparoscopic minimally invasive treatment).Changes in various indicators during the perioperative period between two groups were compared.Results Minimally invasive group had significantly better surgical indicators(surgical duration,intraoperative bleeding,exhaust time,first time out of bed activity,extubation time,length of stay)than open treatment group(P<0.05).Postoperative 1-3dVAS scores was significantly lower than open treatment group(P<0.05).Incidence of complications was significantly lower than laparotomy treatment group(P<0.05).Total effective rate of treatment was significantly higher than open abdominal treatment group(P<0.05).Conclusion Laparoscopic minimally invasive treatment can achieve more significant effect than traditional open surgery,with advantages of less trauma,short recovery time and less bleeding.At the same time,it can reduce perioperative pain and incidence of perioperative complications of patients.
