摘 要:目的 探索注射用乳糖酸红霉素配伍盐酸消旋山莨菪碱注射液,使用5%葡萄糖注射液稀释时,为保证成品输液质量,在调配过程中5%碳酸氢钠注射液的最佳用量。方法 通过设置系列浓度梯度试验,检测并记录不同碳酸氢钠浓度下成品输液pH值变化。通过查阅相关文献,比对试验数据,分析使用5%葡萄糖注射液稀释调配乳糖酸红霉素配伍消旋山莨菪碱的最佳5%碳酸氢钠注射液用量。结果 在浓度梯度试验中,添加5%碳酸氢钠注射液0.2mL、0.1mL、0.05mL,放置0h、1h、2h后,成品输液pH值均符合中国药典对乳糖酸红霉素检查项目中酸碱度的要求。通过查阅文献,作者认为添加0.2mL时,试品溶液的pH值各时间段均最接近7.5,为最佳用量。结论 注射用乳糖酸红霉素与盐酸消旋山莨菪碱注射液配伍,使用5%葡萄糖注射液进行稀释调配时,仍需添加碳酸氢钠以调整成品输液的pH值。建议通过浓度梯度试验取得碳酸氢钠最佳用量。Objective To explore optimal dosage of 5%sodium bicarbonate injection during preparation of erythromycin lactobionate for injection and racemic anisodamine hydrochloride injection with dilution of 5%glucose injection,to ensure quality of finished infusion.Methods The paper established a series of concentration gradient tests,detected and recorded the pH changes of finished infusion under different concentrations of sodium bicarbonate.Based on consulting relevant literature and comparing test data,the paper analyzed optimal dosage of 5%sodium bicarbonate injection with dilution of 5%glucose injection to prepare erythromycin lactobionate and racemic anisodamine hydrochloride.Results In concentration gradient test,0.2mL,0.1mL,0.05mL of 5%sodium bicarbonate injection was added with standing time of 0 hour,1 hour and 2 hours.pH values of finished infusion meet requirements of the Chinese Pharmacopoeia for acidity and alkalinity of erythromycin lactobionate test items.Through literature review,the author believes that after adding 0.2mL,the pH value of test solution was close to 7.5 at each time period,which is the optimal dosage.Conclusion Erythromycin lactobionate for injection being compatible with raceanisodamine hydrochloride injection and 5%glucose injection for dilution,sodium bicarbonate can be added to adjust pH value of finished infusion.It is recommended to achieve optimal dosage of sodium bicarbonate through concentration gradient testing.
