益气活血解毒方治疗瘀毒互结型急性重症胰腺炎临床研究

摘　　要：目的:探究益气活血解毒方治疗瘀毒互结型急性重症胰腺炎的疗效。方法:将126例瘀毒互结型急性重症胰腺炎患者随机分为研究组和对照组,每组63例,对照组予以常规西医对症治疗,研究组在对照组基础上联合益气活血解毒方治疗。治疗7 d后,比较两组的中医症候、疗效、血清胰淀粉酶(PAMY)、脂肪酶(LPS)、胃动素(MTL)、胃泌素(GAS)、血管活性肠肽(VIP)、D-乳酸、二胺氧化酶(DAO)、内毒素、白细胞介素-10(IL-10)、单核细胞趋化因子蛋白-1(MCP-1)、核因子κB p65(NF-κB p65)水平,记录不良反应。结果:治疗后,两组患者的主症评分、次症评分均降低,研究组低于对照组(P<0.05)。研究组治疗总有效率高于对照组(P<0.05)。治疗后,两组患者的PAMY、LPS、GAS、D-乳酸、DAO、内毒素、MCP-1、NF-κB p65水平均低于治疗前,研究组低于对照组(P<0.05);MTL、VIP、IL-10水平均高于治疗前,研究组高于对照组(P<0.05)。两组患者均未出现严重不良反应。结论:益气活血解毒方治疗瘀毒互结型急性重症胰腺炎,可有效改善患者的胃肠道功能,抑制炎症反应,提升临床疗效。Objective:To exploring the therapeutic effect of Yiqi Huoxue Jiedu formula on acute severe pancreatitis with blood stasis and toxin accumulation.Methods:A total of 126 cases of acute severe pancreatitis with the combination of blood stasis and toxin were randomly divided into a study group and a control group,with 63 cases in each group.The control group received routine western medicine symptomatic treatment,while the study group was treated with Yiqi Huoxue Jiedu formula on the basis of the control group.After 7 days of treatment,the TCM syndromes,efficacy,serum levels of pancreatic amylase(PAMY),lipase(LPS),motilin(MTL),Gastrin(GAS),vasoactive intestinal peptide(VIP),D-lactic acid,diamine oxidase(DAO),endotoxin,interleukin-10(IL-10),monocyte chemoattractant protein-1(MCP-1),and nuclear factor-κB p65(NF-κB p65)were compared between the two groups,and adverse reactions were recorded.Results:After treatment,the main and secondary syndrome scores of both groups of patients decreased,and the study group was lower than the control group(P<0.05).The effective rate of treatment in the research group was higher than that in the control group(P<0.05).After treatment,the levels of PAMY,LPS,GAS,D-lactate,DAO,endotoxin,MCP-1,and NF-κB p65 in both groups of patients were lower than before treatment,and the study group was lower than the control group(P<0.05).The levels of MTL,VIP,and IL-10 were all higher than before treatment,and the study group was higher than the control group(P<0.05).Both groups of patients did not experience serious adverse reactions.Conclusion:The Yiqi Huoxue Jiedu formula can effectively improve patients’gastrointestinal function,inhibit inflammatory reactions,and enhance clinical efficacy in the treatment of acute severe pancreatitis with the combination of blood stasis and toxin.
