胡荫奇教授病证结合经方辨证治疗未分化脊柱关节病的临床经验

摘　　要：未分化脊柱关节病(Undifferentiated spondyloarthritis,USpA)症状繁多,早期缺乏明确的诊断证据,现代医学无法进行精准药物干预,易错失最佳治疗时机,造成病情延误,影响患者生活质量。胡荫奇教授是国家名老中医、首都国医名师,行医近60载,对USpA研究极为深入,提出“病证结合、分期论治”USpA,擅于辨证运用经方加减,疗效颇佳。胡教授认为急性期湿热毒瘀痹阻筋脉为病机关键,外邪内侵,日久不去,化热蕴毒,瘀阻经络而生诸症;缓解期则以肝脾肾三脏亏虚为病机关键,正气亏虚,邪气乘虚而入,痹阻经脉而成。治疗上急性期以清热除湿、化瘀通络为主,缓解期以补肾固本为主,兼调肝脾,遣方用药以经方为基础,加减配伍精妙灵活,并坚持中和之道,有效治疗USpA,同时为USpA提供新的治疗途径。The symptoms of undifferentiated spondyloarthritis(USpA) are diverse,and there is a lack of clear diagnostic evidence in the early stages.Modern medicine cannot provide precise drug interventions,which can lead to missed opportunities for optimal treatment,resulting in delayed disease progression and impacting the patient′s quality of life.Professor Hu Yinqi is a renowned traditional Chinese medicine(TCM)doctor,a nationally recognized senior TCM expert,and a renowned TCM doctor in Beijing.With nearly 60 years of clinical experience,he has conducted in-depth research on USpA and proposed the approach of “treating the disease based on syndrome differentiation by stages”.He excels in usingclassic herbal formulas with modifications based on syndrome differentiation and has achieved remarkable treatment efficacy.Professor Hu believes that in the acute phase,the key pathological mechanism lies in the obstruction of the muscles and tendonsby dampness-heat and blood stasis.Prolonged invasion of external pathogenic factors leads to the accumulation of heat and toxins,resulting in the blockage of meridians and the manifestation of various symptoms.In the remission phase,the key pathological mechanism lies in the deficiency of the liver,spleen,and kidneys.Deficient healthy qi allows pathogenic factors to invade,leading to the blockage of meridians and the development of the condition.The treatment approach focuses on clearing heat,eliminating dampness,resolving blood stasis,and dredging collateralsin the acute phase,while in the remission phase,the emphasis is on nourishing the kidneys and strengthening the foundation,along with regulating the liver and spleen.Professor Hu′s treatment method is based on herbal formulas,with skillful modifications and compatibility,and emphasizes the principle of harmonizing and neutralizing.This approach effectively treats USpA and provides new treatment options for this disease.
