摘  要:目的 探讨芪石升降归元颗粒对胃食管反流病(Gastroesophageal reflux disease,GERD)相关肺部及气道炎症反应的影响。方法 将40只健康SPF级SD大鼠,采用随机数字表法分为对照组、模型组、中药组、西药组,每组各10只。采用食管下端盐酸灌注法制备GERD合并肺部炎症大鼠模型,光镜下观察食管下端、气管近候部及肺组织形态,伊文思蓝浸入法测定肺毛细血管通透性,酶联免疫吸附法(Enzyme linked immunosorbent assay,ELISA)检测血清肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白细胞介素-5(Interleukin-5,IL-5)及白细胞介素-10(Interleukin-10,IL-10)表达水平。结果 与对照组比较,模型组大鼠食管下端、气道近喉部、肺组织炎症细胞浸润;与模型组比较,中药组、西药组食管下端、气道近喉部、肺组织炎症细胞浸润明显减少。与对照组比较,模型组大鼠肺毛细血管的伊文思蓝染色明显升高,差异有统计学意义(P<0.05),与模型组比较,中药组、西药组大鼠肺毛细血管的伊文思蓝染色明显减少,差异有统计学意义(P<0.05)。与对照组比较,模型组大鼠血清TNF-α、IL-5表达增加,IL-10表达减少,差异有统计学意义(P<0.05);与模型组比较,中药组及西药组大鼠血清TNF-α、IL-5表达减少,IL-10表达增加,差异有统计学意义(P<0.05)。结论 芪石升降归元颗粒缓解GERD相关肺部及气道炎症反应的作用机制可能与抑制TNF-α、IL-5的产生,促进IL-10生成,抑制肺部、气道及食管炎症细胞浸润,减少炎症反应,提高肺通气功能相关。Objective To explore the effect of Qishi Shengjiang Guiyuan Granules on pulmonary and airway inflammatory responses related to gastroesophageal reflux disease(GERD).Methods Forty healthy SPF-grade SD rats were randomly divided into the following four groups:a control group,a model group,a Chinese medicine group,and a western medicine group,with 10 rats in each group.GERD combined with pulmonary inflammation rat model was established using lower esophageal acid perfusion.Microscopic observations were made on the morphology of the lower esophagus,trachea proximal to the larynx,and lung tissues.The Evans Blue dye method was employed to measure pulmonary capillary permeability,and enzyme-linked immunosorbent assay(ELISA)was conducted to detect serum levels of tumor necrosis factor-α(TNF-α),interleukin-5(IL-5),and interleukin-10(IL-10).Results Compared with the control group,the model group exhibited inflammation cell infiltration in the lower esophagus,trachea proximal to the larynx,and lung tissues.Compared with the model group,both the Chinese medicine group and the western medicine group showed a significant reduction in inflammation cell infiltration in these areas.Compared with the control group,the model group showed significantly increased Evans Blue staining in pulmonary capillaries(P<0.05).However,the Chinese medicine and western medicine groups displayed a significant decrease in Evans Blue staining in pulmonary capillaries compared with the model group(P<0.05).Compared with the control group,the model group had increased expression of serum TNF-α and IL-5,and decreased expression of IL-10(P<0.05).However,both the Chinese medicine and western medicine groups showed decreased expression of serum TNF-α and IL-5,along with increased expression of IL-10 compared with the model group(P<0.05).Conclusion The mechanism of Qishi Shengjiang Guiyuan Granules in alleviating GERD-related pulmonary and airway inflammatory responses may involve inhibiting the production of TNF-α and IL-5,promoting IL-10 generation,su
