摘　　要：目的 探讨手法复位、小夹板外固定联合中药薰洗保守治疗桡骨远端骨折的临床效果。方法 选取2020年6月至2022年6月在泉州市正骨医院正骨科治疗的86例桡骨远端骨折患者,采用随机数表法进行分组,对照组使用手法复位、小夹板外固定,观察组在此基础上联合中药薰洗保守治疗,比较两组腕关节功能恢复效果、各项治疗指标、疼痛及肿胀指标、骨折愈合指标。结果 观察组腕关节功能优良率为93.02%,明显高于对照组的74.42%,差异有统计学意义(P <0.05);观察组肿胀消退时间、疼痛消失时间、骨折愈合时间均短于对照组,差异有统计学意义(P <0.05);观察组治疗3、7、14 d的视觉模拟评分法(VAS)疼痛评分、腕关节肿胀评分均低于对照组,差异有统计学意义(P <0.05);观察组治疗4周的桡骨掌倾角、尺偏角均大于对照组,桡骨相对高度高于对照组,差异有统计学意义(P <0.05)。结论 手法复位、小夹板外固定联合中药薰洗保守治疗桡骨远端骨折的临床效果显著,能促进疼痛及肿胀缓解,缩短骨折愈合时间,提高骨折愈合质量。Objective To investigate the clinical effect of manual reduction,external fixation with small splints combined with Chinese medicine herbal fumigation on the conservative treatment for distal radius fractures.Methods A total of 86 patients with distal radius fractures treated in the Department of Bone Orthopedics of Quanzhou Orthopedic-Traumatological Hospital from June 2020 to June 2022 were randomly divided into two groups using the random number table method.The control group was treated with manual reduction and external fixation with small splints.On this basis,the observation group was treated with Chinese medicine herbal fumigation for conservative treatment.The recovery effect of wrist joint function,various treatment indicators,pain and swelling indicators,and fracture healing indicators of the two groups were compared.Results The excellent and good rate of wrist joint function in the observation group was 93.02%,significantly higher than 74.42%in the control group,with a statistically significant difference(P<0.05).The observation group had shorter swelling resolution time,pain resolution time,and fracture healing time compared to the control group,with statistically significant differences(P<0.05).The visual analogue scale(VAS)pain score and wrist joint swelling score of the observation group were lower than those of the control group after treatment for 3 days,7 days,and 14 days,with statistically significant differences(P<0.05).After 4 weeks of treatment,the radial metacarpal angle and ulnar deviation angle in the observation group were greater than those in the control group,and the relative height of the radius was higher than that of the control group,with statistically significant differences(P<0.05).Conclusion The clinical effect of manual reduction,external fixation with small splints combined with Chinese medicine herbal fumigation on the conservative treatment for distal radius fractures is significant,which could promote pain and swelling relief,shorten fracture healing time,and improve the
