高选择性翼管神经切断术治疗常年性变应性鼻炎的疗效评估

摘　　要：目的 评估分析高选择性翼管神经切断术应用于常年性变应性鼻炎治疗的临床效果。方法 从常年性变应性鼻炎患者中选取100例,随机分为对照组和研究组各50例。对照组采取常规功能性鼻内镜鼻-鼻窦手术治疗,研究组采取内镜辅助下的高选择性翼管神经切断术(眶支保留)。分别采用鼻部症状视觉模拟评分(VAS)和鼻结膜炎生存质量调查问卷(RQLQ)统计分析2组患者术前及术后生活质量、鼻阻塞、打喷嚏、流涕等症状严重程度,从而分析两种治疗方式的疗效差异。结果 手术前两组患者的鼻炎症状VAS评分及RQLQ评分差异无统计学意义(P>0.05);而手术后的鼻炎症状VAS评分及RQLQ评分对照组均高于研究组,两组比较差异有统计学意义(P<0.01)。结论 对常年性变应性鼻炎病例,相较于常规功能性鼻内镜鼻-鼻窦手术,高选择性翼管神经切断术是更为有效的手术治疗方式。Objective To investigate the clinical efficacy of highly selective pterygoid canal neurotomy in the treatment of perennial allergic rhinitis.Methods 100 patients with perennial allergic rhinitis were randomly selected and divided into a control group and a study group,with 50 patients in each group.The control group received conventional functional endoscopic nasal cavity-sinus surgery,while the study group received endoscopic assisted highly selective pterygoid neurotomy(Preserve the orbital branch of the pterygoid canal nerve).The nasal symptom visual analogue scale(VAS)and rhinoconjunctivitis quality of life questionnaire(RQLQ)were used to statistically analyze the improvement of preoperative and postoperative quality of life,nasal congestion,sneezing,and runny nose in two groups of patients,and to analyze the difference in efficacy between the two treatment methods.Results There was no statistically significant difference in the VAS score and RQLQ score of rhinitis symptoms between the two groups of patients before surgery(P>0.05);The VAS score and RQLQ score of postoperative rhinitis symptoms in the control group were higher than those in the study group,with a statistically significant difference between the two groups(P<0.001).Conclusion For perennial Allergic rhinitis cases,highly selective pterygoid neurotomy is a more effective surgical treatment compared to conventional functional endoscopic nasal cavity-sinus surgery.
