摘 要:新生儿缺氧缺血性脑病(hypoxic-ischemic encephalopathy,HIE)是围生期因缺氧导致的脑部缺氧缺血性损伤,是新生儿期常见的神经系统疾病之一,治疗不及时可引起严重的并发症及后遗症,包括不良的神经发育结局及死亡。目前该疾病暂未发现特效治疗药物,因此早期进行临床干预十分重要。该文主要综述颅脑超声、振幅整合脑电图和近红外光谱对新生儿HIE诊断及脑功能监测的优缺点,以及对便携式核磁共振应用的展望,以期为HIE患儿提供脑功能监测及早期诊疗,更好地预测患儿近期和远期结局。Neonatal hypoxic-ischemic encephalopathy(HIE)is a perinatal hypoxic ischemic injury caused by hypoxia,which is one of the common neurological disorders in neonatal period,and the untimely treatment can lead to serious complications and sequelae,including poor neurodevelopmental outcome and death.Currently,no specific therapeutic drugs have been found for this disease,so early clinical intervention is very important.This article reviews the advantages and disadvantages of cranial ultrasound,amplitude-integrated electroencephalography,and near-infrared spectroscopy for the diagnosis of neonatal HIE and the monitoring of brain function,as well as the outlook for the use of portable magnetic resonance.The aim is to provide cerebral function monitoring and early diagnosis and treatment for children with HIE,and to better predict their immediate and long-term outcomes.
