宫腔镜诊疗宫腔因素不孕不育的价值及对生殖预后的影响研究

摘　　要：目的 研究宫腔镜在宫腔因素导致不孕不育患者诊疗中的临床应用价值。方法 选取2017年5月-2020年6月驻马店市中医院收治的由于宫腔因素所导致不孕不育患者80例,所有患者在宫腔镜下检查及治疗,对患者进行6个月~1年的随访,随访采用电话或微信的方式,比较治疗前后的宫腔因素及治疗后的生殖预后情况。结果 治疗前宫腔病变因素有子宫纵膈8例,宫腔黏连12例,子宫黏膜下肌瘤5例,子宫内膜息肉15例,宫腔镜治疗后子宫纵膈、宫腔黏连、子宫黏膜下肌瘤及子宫内膜息肉分别为1例、2例、1例、3例,治疗后均明显减少,获得显著改善(P<0.05);宫腔镜治疗后不孕不育患者妊娠率根据不同病因分别为:子宫纵膈妊娠率为50.0%,宫腔黏连患者妊娠率为83.3%,子宫黏膜下肌瘤患者妊娠率为80.0%,子宫内膜息肉患者妊娠率为100.0%。结论 宫腔镜在宫腔内病变导致的不孕不育症患者检查及治疗方面均具有非常高的价值,且临床治疗效果较好,可在临床推广应用。Objective To study the clinical application value of hysteroscopy in the diagnosis and treatment of infertility patients caused by uterine factors.Methods From May 2017 to June 2020,80 patients with infertility caused due to u-terine factors admitted to our hospital,all of the patients under hysteroscopy examination and treatment,the patients with fllow-up of 6 months to 1 year,follow-up,adopt the way of telephone or WeChat comparison before and after the treatment of uterine factors and the reproductive prognosis after treatment.Results The factors of uterine cavity lesions before treament have uterine mediastinum 8 cases,12 cases caused by intrauterine adhesion,submucosal uterine fibroids in 5 cases,15 cases of endometrial polyp,uterus after hysteroscopic treatment of the mediastinum,submucosal uterine cavity adhesion,uterine myoma and endometrial polyps were 1 case,2 cases,1 cases,3 cases respectively,were signif-icantly reduced after treatment was improved significantly(P<0.05);the pregnancy rate of infertility patients after hys-teroscopic treatment was 50.0%,83.3%,80.0%and 100.0%respectively according to different causes.Conclusion Hysteroscopy has a very high value in the examination and treatment of infertility patients with intrauterine lesions,and the clinical treatment effect is good,which can be popularized in clinical application.
