糖皮质激素联合四环素及烟酰胺治疗大疱性类天疱疮71例临床疗效观察

摘　　要：目的 观察糖皮质激素联合四环素、烟酰胺治疗大疱性类天疱疮患者71例的临床效果。方法 选取2021年5月-2022年5月郑州大学第一附属医院郑东院区皮肤科收治的大疱性类天疱疮患者142例,采用随机数字表法进行对等分组,对照组(n=71),采用四环素与泼尼松双药联合治疗,研究组(n=71)以四环素、泼尼松以及烟酰胺3药联合治疗,治疗3个月后,以BPDAI指标评分、全身总水疱个数、激素用量、疾病疗效以及不良反应作为观察指标,对比组间临床效果。结果 治疗后,与对照组相比,研究组皮肤、损害、黏膜以及瘙痒评分均明显降低,全身总水疱个数也更少(P<0.05);研究组皮损完全控制时激素用量、激素总用量均较对照组少(P<0.05);研究组治疗总有效率(97.18%)高于对照组(87.32%)(P<0.05);研究组不良反应发生率(7.04%)低于对照组(19.02%)(P<0.05)。结论 四环素、泼尼松以及烟酰胺3种药物联合治疗大疱性类天疱疮效果理想,可有效改善患者的临床症状,减少激素用量,降低不良反应发生风险,值得推广。Objective To observe the clinical effect of tetracycline,prednisone and niacinamide in the treatment of 71 patients with bullous pemphigoid.Methods A total of 142 patients with bullous pemphigoid from the dermatology de-partment of our hospital from May 2021 to May 2022 were selected and divided into equal groups by random number table method.The control group(n=71)was treated with tetracycline combined with prednisone,and the study group(n=71)was treated with tetracycline combined with prednisone and niacinamide.After 3 months of treatment,BPDAI index score,total number of blisters,hormone dosage,disease efficacy and adverse reactions were used as observation indexes to compare the clinical effects between groups.Results After treatment,compared with the control group,the scores of skin,lesion,mucosa and pruritus were significantly decreased in the study group,and the total number of blisters was also less(P<0.05).The hormone dosage and total hormone dosage in the study group were lower than those in the control group(P<0.05).The total effective rate of the study group(97.18%)was higher than that of the control group(87.32%)(P<0.05).The incidence of adverse reactions in the study group(7.04%)was lower than that in the control group(19.02%)(P<0.05).Conclusion The combination of tetracycline,prednisone and niacinamide has an ideal effect in the treatment of bullous pemphigoid,which can effectively improve the clinical symptoms of pa-tients,reduce the dosage of hormones,and reduce the risk of adverse reactions.It is worth promoting.
