浅述脾虚湿盛型泄泻中医辨治思路

摘　　要：脾虚湿盛型是泄泻中最常见的症型之一,就近年来相关的临床研究文献进行梳理,总结并提出新的中医辨治思路。脾虚湿盛型泄泻传统的中医内治法以健脾、渗湿为主,外治法以脐灸、穴位贴敷、灌肠等为主,而内外联合治疗是当今中医药治疗的研究热点与趋势。在临床实践中另辟蹊径,予以升阳止泻法治疗脾虚湿盛型泄泻疗效同样显著,并且发现联合应用升阳除湿法内涵组方的隔药灸脐法疗效更佳,通过使阳气升、浊气降,升降恢复有序达到止泻的效果。故提出升阳止泻法可以用于治疗脾虚湿盛型泄泻,升阳止泻法不但是内服中药组方用药的法则,而且还是隔药灸脐法的核心,临床疗效显著。基于此,阐述升阳止泻法的内涵及病机,并结合临床经验来论证其有效性,为脾虚湿盛型泄泻的中医治疗提供了一个新思路。Damp abundance due to spleen deficiency is one of the most common syndrome types in diarrhea.This paper combs the TCM internal treatment of diarrhea with damp abundance due to spleen deficiency is mainly to strengthen the spleen and infiltrate dampness,while the external treatment is mainly umbilical moxibustion,acupoint application and enema.The combination of internal and external treatment is the research hotspot and trend of TCM treatment.In clinical practice,the author found a new way to treat diarrhea of damp abundance due to spleen deficiency with the method of ascending yang and stopping diarrhea.It was also found that herb-partitioned moxibustion on umbilicus combined with the connotation of ascending yang and dehumidification was more effective.The effect of stopping diarrhea was achieved by raising yang qi,lowering turbid qi and restoring orderly rise and fall.Therefore,this paper proposes that the method of ascending yang and stopping diarrhea can be used to treat diarrhea of damp abundance due to spleen deficiency.The method of ascending yang and stopping diarrhea is not only the law of oral TCM,but also the core of herb-partitioned moxibustion on umbilicus,and the clinical effect is remarkable.This paper expounds the connotation and pathogenesis of the method of ascending yang and stopping diarrhea,and demonstrates its effectiveness combined with clinical experience,so as to provide a new idea for the TCM treatment of diarrhea of damp abundance due to spleen deficiency.
