委省共建国家医学中心和国家区域医疗中心协议落实情况分析

摘　　要：目的分析委省共建国家医学中心和国家区域医疗中心的协议落实情况,以期为委省共建推动双中心建设提供相关建议。方法制作调查表收集14个省(市)落实委省共建协议的文本资料,利用目标对应法,采用Likert5级评分法对落实程度进行打分。结果从集中度来看,省级政府层面集中度较高的条款是“将双中心建设纳入本省(市)社会发展总体规划和改革重点内容”等3条;异质化较高的条款是“整合高校附属医院资源,加强重点学科服务能力建设”。高校层面各项职责条款的集中度均较高。从落实程度来看,省级政府职责落实程度较好的是“加大对委省共建医院财政投入和政策支持力度”;得分较低的条款是“对共建医院参加属地相关事业单位养老保险改革给予政策倾斜和经费支持”。从落实进度来看,各省(市)及高校在各条款上的落实进度均有所差异。结论委省共建协议落实重点在于外部治理机制的完善,难点在于推动形成适宜于“双中心”经济运行的新机制,高校与其附属医院在医教研管协同发展方面仍需加强。Objective Analyze the implementation of the agreement of National Medical Center and Regional Medical Center constructed by the National Health Commission and provinces,with the aim of providing relevant suggestions for the construction of the double center.Methods Questionnaire was made to collect and analyze the text data of the implementation of the co-construction agreements in 14 provinces,the target matching method and 5-level Licott score were used to score its implementation degree.Results The three clauses with high concentration at the provincial government level are in three cluding the disciplines construction of two centers into the overrall plan for social develepment and key reform contents of the province etc.The more disparate clauses are“integrating the resources of university affiliated hospitals,strengthening the construction of serrice ability of key disciplines”.The concentration of various clauses at the university level is high.The clauses with better implementation degree at provincial government level are“increasing financial investment and policy support”.The clauses with lower scores are“giving preferential policy and financial support to co-build hispitals to participate in the pension insurance reform of territorial organs and public institutions”.In terms of implementation progress,provinces and universities are different.Conclusion The key to promoting the implementation of the co-construction agreement lies in the improvement of external governance mechanism,while the difficulty lies in the formation of an operational mechanism suitable for the national central economy.The collaborative development of medical treatment,teaching,researches and management between universities and their affiliated hospitals still needs to be strengthened.
