摘 要:目的研究慢性阻塞性肺疾病(简称慢阻肺)合并左室舒张功能不全(LVDD)与炎症因子水平的相关性及机制。方法招募2021年1月~2021年12月符合入组条件的慢阻肺患者49例,其中男33例,女16例,平均年龄(72±8)岁。所有入组对象均采集一般临床资料、外周血标本,给予肺功能检查、心脏超声检查。根据心脏超声检查结果将患者分为慢阻肺合并LVDD组(40例)、慢阻肺不合并LVDD组(9例)。分析并比较两组的炎症因子水平。结果两组的IL-6分别为4.98(3.53,7.95)pg/ml、2.38(2.00,5.36)pg/ml,慢阻肺合并LVDD组的IL-6明显高于慢阻肺不合并LVDD组(P<0.05);两组的TNF-α分别为11.10(7.83,21.40)pg/ml、7.25(6.39,9.71)pg/ml,慢阻肺合并LVDD组的TNF-α明显高于慢阻肺不合并LVDD组(P<0.05)。慢阻肺合并LVDDⅠ级组的IL-6、TNF-α水平低于II级组,差异均有统计学意义(P<0.05)。慢阻肺合并LVDD患者LVDD分级与IL-6(r=0.394,P<0.05)、TNF-α(r=0.402,P<0.05)均呈正相关。结论慢阻肺合并LVDD与炎症因子水平升高相关。Objective To study the correlation and mechanism between inflammatory factors and chronic obstructive pulmonary disease(COPD)combined with left ventricular diastolic dysfunction(LVDD).Methods 49 patients with COPD from January 2021 to December 2021 who met the inclusion criteria were recruited,including 33 males and 16 females,average aged(72±8)years.General clinical data,peripheral blood samples were collected,pulmonary function examination and echocardiography were performed from all enrolled subjects.According to echocardiography,the patients were divided into COPD combined with LVDD group(40 cases)and COPD without LVDD group(9 cases).The levels of inflammatory factors in the two groups were analyzed and compared.Results The IL-6 of the two groups were 4.98(3.53,7.95)pg/ml and 2.38(2.00,5.36)pg/ml,respectively.The IL-6 of the COPD combined with LVDD group was significantly higher than that of the COPD without LVDD group(P<0.05).The TNF-αof the two groups were 11.10(7.83,21.40)pg/ml and 7.25(6.39,9.71)pg/ml,respectively.The TNF-αof the COPD combined with LVDD group was significantly higher than that in COPD without LVDD group(P<0.05).The levels of IL-6 and TNF-αin the COPD combined with LVDD grade I group were lower than those in the grade II group,and the difference was statistically significant(P<0.05).The LVDD grade of COPD patients with LVDD was positively correlated with IL-6(r=0.394,P<0.05)and TNF-α(r=0.402,P<0.05).Conclusion COPD combined with LVDD is related to the increase levels of inflammatory factors.
