血清CD40L、VCAM-1、sTREM-1水平与重症肺炎患者APACHEⅡ评分的相关性

摘　　要：目的:分析血清CD40配体(CD40L)、血管细胞黏附分子-1(VCAM-1)、可溶性髓样细胞触发受体-1(sTREM-1)水平与重症肺炎患者急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分的相关性。方法:选取2020年3月至2021年6月该院收治的92例重症肺炎患者为研究对象,纳入研究组。另选取同期本院110名健康体检者为对照组。比较两组和不同APACHEⅡ评分(高危、中危、低危)患者血清CD40L、VCAM-1、sTREM-1水平,采用Pearson相关性分析血清CD40L、VCAM-1、sTREM-1水平与重症肺炎患者APACHEⅡ评分的相关性。结果:研究组血清CD40L、VCAM-1、sTREM-1水平均高于对照组,差异有统计学意义(P<0.05);高危患者血清CD40L、VCAM-1、sTREM-1水平均高于中危、低危患者,且中危患者高于低危患者,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,血清CD40L、VCAM-1、sTREM-1水平与重症肺炎患者APACHEⅡ评分均呈正相关(r>0,P<0.05)。结论:血清CD40L、VCAM-1、sTREM-1水平与重症肺炎患者APACHEⅡ评分均呈正相关。Objective:To analyze correlations among serum levels of CD40 ligand(CD40L),vascular cell adhesion molecule-1(VCAM-1),soluble triggering receptor expressed on myeloid cells-1(sTREM-1)and acute physiology and chronic health evaluation II(APACHE II)score in patients with severe pneumonia.Methods:92 patients with severe pneumonia admitted to the hospital from March 2020 to June 2021 were selected as the study subjects and included in the study group.Another 110 healthy subjects in this hospital during the same period were selected as the control group.The levels of serum CD40L,VCAM-1 and sTREM-1 were compared between the two groups and among the patients with different APACHE II scores(high-risk,medium-risk and low-risk).Pearson correlation analysis was used to analyze the correlations among the serum CD40L,VCAM-1,sTREM-1 levels and the APACHE II score in the patients with severe pneumonia.Results:The levels of serum CD40L,VCAM-1 and sTREM-1 in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of serum CD40L,VCAM-1 and sTREM-1 in the high-risk patients were higher than those in the medium-risk patients and low-risk patients,those in the medium-risk patients were higher than those in the low-risk patients,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the serum levels of CD40L,VCAM-1 and sTREM-1 were positively correlated with the APACHE II score in the patients with severe pneumonia(r>0,P<0.05).Conclusions:The levels of serum CD40L,VCAM-1 and sTREM-1 are positively correlated with the APACHE II score in the patients with severe pneumonia.
