三维斑点追踪成像应变指标绝对值与冠心病患者PCI术前心功能指标水平、心功能分级的相关性

摘　　要：目的:分析三维斑点追踪成像(3D-STI)应变指标绝对值与冠状动脉粥样硬化性心脏病(冠心病)患者经皮冠状动脉介入(PCI)术前心功能指标水平、心功能分级的相关性。方法:选取2018年4月至2020年8月该院收治的87例冠心病患者为研究对象,纳入观察组,并择期行PCI术治疗,另选取同期该院健康体检者84名为对照组。两组均行3D-STI检查,比较两组心功能指标[左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、左心室射血分数(LVEF)]水平,PCI术前不同心功能分级患者3D-STI应变指标[左心室收缩期峰值纵向应变(GLS)、圆周应变(GCS)、面积应变(GAS)、径向应变(GRS)]绝对值,采用Pearson相关性分析3D-STI应变指标绝对值与冠心病患者PCI术前心功能指标水平、心功能分级的相关性。结果:观察组LVEF水平低于对照组,LVESV、LVEDV水平均高于对照组,差异有统计学意义(P<0.05);观察组GRS、GAS、GCS、GLS绝对值均低于对照组,差异有统计学意义(P<0.05);心功能Ⅱ级患者GRS、GAS、GCS、GLS绝对值均高于Ⅲ级、Ⅳ级患者,且Ⅲ级患者均高于Ⅳ级患者,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,GRS、GAS、GCS、GLS绝对值与冠心病患者PCI术前心功能分级、LVESV水平、LVEDV水平均呈负相关(r<0,P<0.05),GRS、GAS、GCS、GLS绝对值与冠心病患者PCI术前LVEF水平均呈正相关(r>0,P<0.05)。结论:3D-STI应变指标绝对值与冠心病患者PCI术前心功能分级、LVESV水平、LVEDV水平均呈负相关,与LVEF水平呈正相关。Objective:To analyze correlations among absolute values of three-dimensional speckle tracking imaging(3D-STI)strain indexes and cardiac function index levels and cardiac function classification in patients with coronary heart disease before percutaneous coronary intervention(PCI).Methods:87 patients with coronary heart disease admitted to the hospital from April 2018 to August 2020 were selected as the research objects and included in the observation group,and they were treated with elective PCI.In addition,84 healthy subjects in the hospital during the same period were selected as the control group.Both groups underwent 3D-STI examination.The levels of cardiac function indexes[left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV)and left ventricular ejection fraction(LVEF)]were compared between the two groups.The absolute values of 3D-STI strain indexes[left ventricular systolic peak longitudinal strain(GLS),circumferential strain(GCS),area strain(GAS)and radial strain(GRS)]in the patients with different cardiac function classifications before PCI were also compared.Pearson correlation analysis was used to analyze the correlations among the absolute value of 3D-STI strain indexes and the level of cardiac function index and cardiac function classification in the patients with coronary heart disease before PCI.Results:The LVEF level of the observation group was lower than that of the control group,the levels of LVESV and LVEDV were higher than those of the control group,and the differences were statistically significant(P<0.05).The absolute values of GRS,GAS,GCS and GLS in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The absolute values of GRS,GAS,GCS and GLS in the patients with cardiac function classification II were higher than those with classification III and classification IV,those of the patients with classification III were higher than those of the patients with classification IV,and the differ
