摘　　要：目的探讨阴茎背侧根部两点固定联合包皮环切术治疗小儿隐匿阴茎的效果。方法回顾性分析2020-06—2021-10泉州市第一医院小儿外科收治的40例隐匿阴茎患儿的临床资料。年龄8.3岁(范围:3~12岁)。所有患儿均采用一次性包皮环切吻合器行阴茎背侧根部两点固定联合包皮环切术。结果40例患儿均顺利完成手术。手术时间21.0 min(范围:15~25 min),术中出血量1.5 mL(范围:1~2 mL),术后住院时间2.0 d(范围:1~3 d)。术后保留阴茎体敷料时间5.0 d(范围:4~6 d)。术后随访6~22个月,阴茎外露延长值2.1cm(范围:1.8~2.6 cm)。无阴茎回缩及包皮堆积,瘢痕与包皮环切一致。10例患儿出现轻度的包皮水肿,均于4~8周完全消退。40例中,35例患儿家长对手术表示满意,5例表示不满意,满意率为87.5%。结论采用一次性包皮环切吻合器行阴茎背侧根部两点固定联合包皮环切术治疗小儿隐匿阴茎,操作简单、无需进行阴茎脱套,创伤小,术后阴茎显露好、外形美观,回缩率低、并发症少,家长满意度高。Objective To investigate the effect of two-point fixation at the dorsal root of the penis combined with circumcision in the treatment of concealed penis in children.Methods The clinical data of 40 children with concealed penis admitted to our department from June 2020 to October 2021 were retrospectively analyzed.The children age ranged from 3 years to 12 years,with an average of 8 years and 3 months.All the children were surgically treated with two-point fixation at the dorsal root of the penis combined with circumcision(disposable circumcision stapler).Results All 40 patients successfully completed the operation,and the operation time was 15~25min,average 21min.Intraoperative blood loss was 1~2 mL,with an average of 1.5 mL.The patients were discharged from the hospital 1~3 days after the operation,with an average of 2 days.The penile body dressing was removed 4~6 days after operation,with an average of 5 days.Follow-up from 6 months to 22 months after operation showed that the extension of penis exposure was 1.8~2.6 cm,with an average of 2.1 cm.There was no penis retraction,no prepuce accumulation,and postoperative scars were consistent with circumcision,which was more beautiful.Only 10 children had mild foreskin edema,no severe foreskin edema,the edema completely subsided in 4~8 weeks.Among the 40 cases,parents were satisfied with the operation in 35 cases and dissatisfied in 5 cases,with a satisfaction rate of 87.5%.Conclusion The treatment of concealed penis by two-point fixation of dorsal root of penis combined with circumcision has the advantages of simple operation,no need for penile degloving,small intraoperative trauma,good postoperative penile exposure,beautiful appearance,low retraction rate,less complications and high parental satisfaction.
