摘 要:目的比较血清微小-RNA-142-3p(microRNA-142-3p,miR-142-3p)联合白细胞介素-1β(interleukin-1 beta,IL-1β)检测与腰椎磁共振弥散加权成像(diffusion weighted imaging,DWI)技术在辅助诊断腰椎间盘退变中的应用效果。方法以2020年6月~2021年6月苏州大学附属苏州九院收治的48例腰椎间盘突出症患者为退变组,同期在院健康体检的志愿者41例为对照组。在MR常规T2WI序列上,对腰椎间盘按Pfirrmann标准进行形态学分级,在矢状面DWI图上对正中层面椎间盘进行表观弥散系数(apparent diffusion coefficient,ADC)测量。测定两组血清miR-142-3p、IL-1β水平,分析不同检查方式的诊断效果。结果退变组患者的ADC值及血清miR-142-3p相对表达量低于对照组,而IL-1β水平高于对照组,差异均具有统计学意义(P<0.05)。Pearson相关性分析显示,miR-142-3p与ADC值呈正相关(r=0.583,P<0.01);IL-1β与ADC值呈负相关(r=-0.768,P<0.01)。Spearman相关性分析显示,miR-142-3p与Pfirrmann(PM)分级呈负相关(r=-0.713,P<0.01);IL-1β与PM分级呈正相关(r=0.608,P<0.01)。血清miR-142-3p、IL-1β检测,二者联合检测及腰椎间盘ADC值预测腰椎间盘退变的ROC曲线下面积分别为0.778,0.780,0.864,0.914。结论血清miR-142-3p、IL-1β的表达可能与椎间盘退变发病机制有关,两者联合检测对腰椎间盘退变诊断价值及腰椎间盘ADC值相似,可作为腰椎间盘退变诊断的补充检测手段。Objective To compare the efficacy of serum miR-142-3p and IL-1βlevels with DWI examination of lumbar spine for assessing early intervertebral disc degeneration(IDD).Methods From June 2020 to June 2021,48 patients with lumbar disc herniation(degeneration group)and 41 adult volunteers(control group)were selected as the research objects.Serum miR-142-3p and IL-1βwere measured.Both groups were examined with MRI and DWI of the lumbar spine,and the diagnostic effects of different examination methods were analyzed.Results The apparent diffusion coefficient(ADC)values and the relative expression of miR-142-3p in serum of the degenerative group was lower than that of the control group,while the IL-1βlevel was higher than that of the control group(P<0.05).Pearson correlation analysis showed that miR-142-3p was positively correlated with ADC values(r=0.583,P<0.01),while IL-1βwas negatively correlated with ADC values(r=-0.768,P<0.01).Spearman correlation analysis showed that miR-142-3p was negatively correlated with Pfirrmann(PM)grading(r=-0.713,P<0.01),while IL-1βwas positively correlated with PM grading(r=0.608,P<0.01).For predictive the incidence of early lumbar intervertebral disc degeneration,the area under ROC curve(AUC)of serum miR-142-3p,IL-1β,miR-142-3p combined with IL-1βand ADC value was 0.778,0.780,0.864,0.914 respectively.Conclusion The expression of serum miR-142-3p and IL-1βmay be related to the pathogenesis of lumbar intervertebral disc degeneration,and the combination has a similar diagnostic value for intervertebral disc degeneration compared with ADC values.Serum miR-142-3p and IL-1βdetection can be used as a supplementary means of intervertebral disc degeneration diagnosis.
