伸筋汤熏洗治疗股骨髁骨折术后膝关节僵直疗效分析

摘　　要：目的探讨伸筋汤熏洗治疗股骨髁骨折术后膝关节僵直的临床疗效。方法选取2019年3月至2021年9月郑州市第二人民医院收治的124例股骨髁骨折术后膝关节僵直患者作为研究对象,按照随机数表法将其随机分为观察组(62例)和对照组(62例),观察组患者采用伸筋汤熏洗联合膝关节运动康复器治疗,对照组患者单纯采用膝关节运动康复器治疗,对比观察两组患者膝关节活动度(ROM)等膝关节功能、股骨骨密度及临床疗效。结果治疗4周后,观察组患者膝关节ROM及股骨骨密度均明显大于对照组(t=8.446、39.840,P均<0.001),美国特种外科医院(HSS)评分明显高于对照组(t=10.571,P<0.001);治疗4周后,观察组患者中显效32例、有效26例、无效4例,明显优于对照组患者的显效24例、有效17例、无效21例(Z=-2.627,P=0.009)。结论伸筋汤熏洗可明显改善股骨髁骨折术后膝关节僵直患者的膝关节功能,增加骨密度,疗效显著。Objective To analyze the clinical efficacy of the fumigation and washing with Shenjin Decoction(a traditional Chinese medicine used to nourish muscles and tendons)in the treatment of knee ankylosis after the surgery for femoral condyle fracture.Methods 124 patients with knee ankylosis after the surgery for femoral condyle fracture admitted into Zhengzhou Second People’s Hospital between March 2019 and September 2021 were enrolled as research subjects,and divided into study group(n=62)and control group(n=62)using the random number table.Patients in the study group were treated with the fumigation and washing with Shenjin Decoction plus the knee exercise rehabilitative apparatus,while patients were treated with the knee exercise rehabilitative apparatus alone in the control group.The functions of knee joint including range of motion(ROM)of knee joint,femoral bone mineral density and clinical efficacy of patients were compared between the two groups.Results After 4 weeks of treatment,the ROM of knee joint and femoral bone mineral density of patients were obviously greater in the study group compared with the control group(t=8.446 and 39.840,both P<0.001),and the score of the U.S.Hospital for Special Surgery(HSS)of patients was obviously higher in the study group(t=10.571,P<0.001);the clinical efficacy was evaluated as markedly effective in 32 cases,effective in 26 cases and ineffective in 4 cases in the study group,which was significantly better than that in the control group(Z=-2.627,P=0.009)-markedly effective in 24 cases,effective in 17 cases and ineffective in 21 case.Conclusion The fumigation and washing with Shenjin Decoction can significantly improve the knee functions of patients with knee ankylosis after the surgery for femoral condyle fracture,and increase their bone mineral density,and the clinical efficacy is definite.
